Combined Surgery on the Patients with Traumatic Cataract

摘　　要：目的采用晶状体摘除和前玻璃体切割联合后房型人工晶体植入术治疗外伤性白内障。方法28例(28眼)因眼外伤致外伤性白内障合并眼前段复杂并发症的患者,均行晶状体摘除和前玻璃体切割联合后房型人工晶体植入术,其中4眼同时行后囊膜切除术。术后随访2～25月,观察视力恢复情况。结果术后矫正视力≥0.5者,11眼,占39.29%;视力0.1～0.4者,14眼,占50%;视力<0.1者,3眼,占10.71%。结论一次完成晶状体摘除、前部玻璃体切割和后房型人工晶体植入的联合手术是安全可行的,适应证的选择和熟练的操作技术是手术成功的关键。角膜瘢痕和散光是影响术后视力提高的主要因素。Objective To study the feasibility and safety of the combined surgery of lens removal and anterior vi- trectomy and posterior chamber intraocular lens implantation in the patients with traumatic cataract. Methods Twenty-eight patients(28 eyes)with traumatic cataract and complications of anterior eye segment were treated with combined surgery, Including combined surgery and posterior capsulotomy in 4 eyes. They were followed up for 2~25 months. Results Eleven patients had postoperative correct visual acuity ≥0.5(39.29%),14 cases 0.1~0.4(50%),and 3 cases <0.1(10.71%). Conclusion The results suggest that combined surgery is safe and feasible. Proper selection of indications and skillful manipulation are the key points of combined surgery. Corneal scarring and acquired astigmatism are the major causes affecting postoperative vision acuity.
