摘 要:目的探讨抑癌基因p53和原癌基因C-myc在黏膜相关淋巴组织(MALT)型淋巴瘤中的表达和意义。方法建立包含84例MALT型淋巴瘤、8例DLBCL,160例其他肿瘤及18例淋巴结反应性增生,共252点阵的组织芯片,应用免疫组织化学技术(S-P法)检测p53和C-myc在MALT型淋巴瘤中的表达。结果82例MALT型淋巴瘤中只有18例表达p53,其中阳性细胞表达率>50%的有8例。p53在L-MALT、H-MALT和DLBCL中的表达差异有显著性。淋巴结反应性增生的对照组中,8例有C myc的胞核表达,但只在少数细胞的胞核出现。在恶性程度不同的组织中C myc浆表达有显著性差异,但C myc核表达没有显著性差异。在L MALT、H MALT和DLBCL中,p53的表达与C myc浆表达有正相关关系(r=0.365,P=0.000),与C myc核表达有负相关关系(r=-0.301,P=0.004)。结论在MALT型淋巴瘤中p53并不起关键作用,但p53异常频率的增高与MALT型淋巴瘤向恶性程度高的转变有关。C-myc表达部位的改变与肿瘤恶性程度有关。C-myc胞浆高表达与p53核的高表达可作为判断肿瘤恶性程度增高及预后的指标之一。Objective To explore the expression and significance of antioncogen p53 and protooncogen C-myc in mucosa-associated lymphoid tissues (MALT) lymphoma. Methods A 252-dot tissue microarray consisting of 84 cases of MALT lymphomas,8 cases of diffuse large B cell lymphoma(DLBCL),16 cases of other malignant neoplasia and 18 cases of reactive lymphoid nodes were done. The S-P method was performed to examine the expression of p53 and C-myc. Results p53 expression was in 18 of 82 cases of MALT lymphoma, and the index more than 50% was in eight of 18 cases. There were significant difference among L-MALT, H-MALT and DLBCL. Eight cases of active lympho hyperplasia expressed C-myc in a few of nuclear. The significant difference of C-myc expression was found in cytoplasm in different malignancy, but not in nucleai C-myc. There was positive correlation(r=0.365,P=0.000)among L-MALT、H-MALT and DLBCL between C-myc cytoplasmic expression and p53, otherwise, the negative relationship(r=-0.301,P=0.004)among them between C-myc nuclear and p53 expression. Conclusion p53 does not play an important role in MALT lymphoma. But the malignant degree arise with the index of p53 expression. The expression of C-myc in cytoplasm or in nuclear was related with malignance. The expression of cytoplasmic C-myc and p53 nuclear expression can be regarded as the factor of malignance and prognosis.
