摘 要:目的:探讨Smad4、u PA和PAI- 1在大肠癌中的蛋白表达对大肠癌发生、发展的作用及相互关系。方法:应用免疫组化SP法检测大肠癌5 3例、大肠腺瘤19例、正常大肠粘膜标本11例中Smad4、u PA和PAI- 1蛋白表达情况。结果:Smad4蛋白在大肠癌组的阳性表达显著低于正常大肠粘膜组和腺瘤组,并与大肠癌的组织学分级、淋巴结转移、远处转移、Duke′s分期密切相关(P<0 .0 5 )。u PA和PAI- 1蛋白在大肠癌组的阳性表达显著高于正常大肠粘膜组和腺瘤组,并与大肠癌的组织学分级、浸润深度、淋巴结转移、远处转移、Duke′s分期密切相关(P<0 .0 5 )。Smad4与u PA、PAI- 1在大肠癌中的蛋白表达均呈负相关,u PA与PAI- 1在大肠癌中的蛋白表达呈正相关(P<0 .0 5 )。结论:Smad4、u PA和PAI- 1可能参与了大肠癌发生。Smad4的低表达,u PA和PAI- 1的高表达可能预示着大肠癌组织学分化较差、存在侵袭、转移和较差的预后。u PA和PAI- 1可能是Smad4的下游目的基因。联合检测大肠癌中Smad4、u PA和PAI- 1蛋白的表达情况,相比单一检测一种蛋白而言,对于评估大肠癌的发生、发展及预后更具有意义。Objective:To investigate the mechanism of colorectal carcinogenesis and the clinicopathologic significance of Smad4, uPA and PAI-1 protein expressions in colorectal carcinoma as well as their interaction.Methods:The expressions of Smad4, uPA and PAI-1 protein were detected in 53 cases of carcinomas, 19 cases of adenomas, 11 cases of normal colorectal mucosas by a streptavidin-peroxidase (SP) technique.Results:The positive expressions of Smad4 protein in colorectal carcinomas were much lower than those in normal colorectal mucosas and adenomas, and were associated with histological grades, lymph node metastasis, distant metastasis and Duke′s stage(P<0.05). The positive expressions of uPA and PAI-1 protein in colorectal carcinomas were significantly higher than those in normal colorectal mucosas and adenomas, and were related with histological grades, the depth of invasion, lymph node metastasis, distant metastasis and Duke′s stage (P<0.05). A negative correlation was observed between the expression of Smad4 and that of uPA and PAI-1.A positive correlation was observed between the expression of uPA and that of PAI-1 (P<0.05).Conclusion:Smad4,uPA and PAI-1 might play an important role in carcinogenesis of colorectal carcinoma. Lower expression of Smad4 and overexpression of uPA and PAI-1 in colorectal carcinoma were associated with poorly histological grades, cell migration,tumor metastases and poor prognosis. UPA and PAI-1 might be the target genes of Smad4.It was much more significant to investigate the expression of Smad4,uPA and PAI-1 combinedly in colorectal carcinoma than to investigate the expression of only one of them.
