海洛因依赖患者出现精神症状的临床分析

摘　　要：目的探讨海洛因依赖患者出现精神症状的临床特点。方法选用简明精神病量表筛选出海洛因依赖患者120例,按焦虑抑郁、缺乏活力,激活性、思维障碍和敌对猜疑等5个因子及其等级(A和B级)进行临床分析。结果①120例患者焦虑抑郁因子达100%,其中疑病妄想和自责自罪妄想占25%;②思维障碍因子占52.5%,其中以幻觉和妄想为主占13.3%;③缺乏活力因子占33.3%,其中情感平淡和交流障碍7.5%;④敌对猜疑因子占17.5%,其中由心境抑郁和敌对情绪影响行为的患者4.2%;⑤激活性因子占2.5%。结论根据海洛因依赖患者精神症状的特点有必要拟订合理的治疗方案以解除躯体依赖;采用心理等方式治疗顽固妄想,才能更加有效的实施戒断依赖,使其早日康复回归社会。Objective: To evaluate the clinical features of mental symptoms in diamorphine dependent patients. Methods: 120 diamorphine dependent patients who were selected by brief psychiatric rating scale (BPRS) were analyzed according to 5 factors including apprehension and depression, absence of vigor, motivation, disturbance of thought and suspect hostility and degrees of these elements separated by A and B rank. Results: (1) Of the 120 patients, all had anxious and despondent emotions. 25 % of them reached vain hope level consisting of hypochondriasis and self-guilty mood. (2) Thought barrier accounted for 52.5 % as illusion and wishful thinking made up 13.3 %. (3) Lack of vitality took 33.3 % while indifferent feelings and communication obstacle constituted ~7.5% . (4) Suspect hostility was observed on 17.5 % cases and there were 4.2 % cases having behavior influenced by depressive mood and hostile condition. (5) Activating factor accounted for 2.5 %. Conclusion: As well as removing physical dependence on treating diamorphine dependent patients, psychotherapy should be applied to treat obstinately vain hope. It is necessary to draft reasonable therapy measure according to features of mental symptoms shown on diamorphine dependent patients. Only on this way, can withdrawal treatment carry on effectively and dependants recover and return the society as soon as possible.
