摘 要:目的 探讨肝素酶表达在胃癌发展中的作用。方法 用逆转录聚合酶链反应(RT PCR)测定4 2例胃癌和2 6例非癌组织中肝素酶的表达情况,采用χ2 检验比较肝素酶的表达与胃癌临床病理特征之间的关系。结果 胃癌中有2 6例肝素酶呈阳性表达(阳性率6 1. 9% ) ;非癌组织中只有1例呈弱阳性表达(阳性率3 8% )。肝素酶mRNA在胃癌TNM分期中Ⅲ、Ⅳ期明显比Ⅰ、Ⅱ期表达阳性率高(P <0 . 0 5 ) ,而且更倾向于在有浆膜浸润、淋巴结转移或远端转移的胃癌组织中表达(P <0 . 0 5 )。结论 肝素酶可能促进胃癌浸润和转移,在胃癌的转移途径中,参与淋巴道转移而非血道转移。肝素酶可作为预测胃癌复发、转移的分子标志物之一。Objective To study the roles of heparanase in the development of human gastric carcinomas. Methods Reverse transcription polymerase chain reaction(RT-PCR) was used to measure the expressions of heparanase mRNA in 42 human gastric carcinomas and 26 non-cancerous gastric tissues. Chi-square test was used to analyse the relationships between the mRNA expression of heparanase and the clinicopathological characters in gastric carcinomas. Results The expression of heparanase gene was positive in 26 cases of gastric cancer with a positive rate of 61.9%; While its expression rate in the 26 cases of non-cancerous gastric tissues was 3.8%(1 case, P <0.05). Heparanase mRNA were expressed more higher in Ⅲ, Ⅳ staging than Ⅰ, Ⅱ of gastric carcinomas TNM staging,and it tended to express more in the patients with serosal infiltration, lymph nodes metastases, distance metastases( P <0.05). Conclusion Hepanarase probably enhances infiltration and metastases of gastric carcinomas, it involves in lymphatic way instead of hematogenous way. All these results suggest heparanase as a molecular marker to predicting gastric recrudescence and metastases.
