胃癌患者外周血单个核细胞和肿瘤组织表面分化抗原44变异体6检测的临床意义

摘　　要：目的探讨测定胃癌患者外周血单个核细胞(PBMC)及肿瘤组织细胞表面分化抗原44变异体6(CD44v6)含量(%)的临床意义。方法用流式细胞术(FCM)检测50例胃癌患者术前和术后1周及20名正常人PBMCCD44v6含量(%),以及术后胃癌及癌旁正常组织CD44v6含量。结果胃癌患者术前PBMCCD44v6含量(9.72%±6.65%)比正常对照组(3.11%±1.15%)及术后1周(4.44%±2.33%)明显增高(分别P<0.001和<0.01);胃癌及癌旁正常组织CD44v6含量分别为3.67%±0.63%及1.33%±0.51%(P<0.01);Ⅲ+Ⅳ期胃癌患者PBMC及胃癌组织CD44v6含量(分别为12.56%±6.97%及3.91%±0.87%)均明显高于Ⅰ+Ⅱ期患者(分别为6.89%±3.37%和2.86%±0.65%,均P<0.05)。胃癌淋巴转移患者PBMC及胃癌组织CD44v6含量(分别为11.29%±5.36%和4.41%±0.79%)均明显高于无转移者(7.32%±4.28%和2.61%±0.78%,均P<0.05)。结论用FCM检测胃癌患者PBMCCD44v6含量能反映胃癌组织CD44v6改变,对胃癌诊断、进展与转移评价及术后疗效观察具有重要的临床意义。Objective To investigate the clinical significance of cell surface differentiation antigen 44 (CD44) expression that shared spliced variant 6 (CD44v6)in peripheral blood mononuclear cell(PBMC)and cancerous tissue of patients with gastric carcinoma.Methods Flow cytometry was used to analyze the CD44v6 expression in the primary tumor tissue and these corresponding paracancer normal tissue after gastrectomy and in PBMC at pre-operation and post-operation one week later in 50 patients with gastric cancer and 20 normal controls.Results CD44v6 expression in PBMC was significantly higher in pre-operation of gastric cancer (9.72%±6.65%) than that in both normal controls(3.11%±1.15%) and post-operation one week later (4.44%±2.33%,P<0.001 and P<0.01 respectively). The significant increase (P<0.01) of CD44v6 expression was found in cancerous tissue (3.67%±0.63%) when compared with corresponding paracancer normal tissue of gastric cancer(1.33%±0.51%). The level of CD44v6 expressions in PBMC and cancerous tissue in patients with gastric cancer of Ⅲ ± Ⅳ stage(12.56%±6.97% and 3.91%±0.87% respectively) was significantly higher than that of Ⅰ ±Ⅱ stage(6.89%±3.37% and 2.86%±0.65% respectively,both P<0.05). CD44v6 expressions in PBMC and cancerous tissue in patients with lymph node metastasis (11.29%±5.36% and 4.41%±0.79% respectively)were significantly higher than that of patients without lymph node metastasis(7.32%±4.28% and 2.61%±0.78% respectively,both P<0.05).Conclusions The level of CD44v6 expression in PBMC may be a marker of monitoring the changes of CD44v6 in gastric cancerous tissue and of important clinical value in the diagnosis,judgment of tumor development and metastasis and therapeutic evaluation for the patients with gastric cancer by flow cytometry.
