复合性角膜穿孔伤的联合手术处理

摘　　要：目的　评价复合性角膜穿孔伤施行联合手术处理的方法和效果。方法　对我院住院治疗的复合性角膜穿孔伤67例(67眼)根据角膜穿孔伤的程度及其合并症采取不同的联合手术处理。其中联合晶状体超声乳化吸出+人工晶状体植入15眼;后段玻璃体切除+眼内异物摘出2 7眼;视网膜脱离复位+硅油填充8眼;虹膜修复或瞳孔成形术11眼;晶状体前后囊破裂、玻璃体脱出行前段玻璃体切除术12眼。结果　出院时裸眼视力<0 .0 5者13眼(19.40 % ) ,0 .0 5～0 .2者3 0眼(4 4 .78% ) ,0 .3～0 .4者18眼(2 6.87% ) ,0 .5～1.0者6眼(8.96% )。本组除较好保住眼球形状,瞳孔恢复圆形或基本圆形外,视功能也得到不同程度提高。结论　联合手术处理复合性角膜穿孔伤,一次手术可完成二联或三联甚至四联手术,可缩短病程,疗效良好。Objective To evaluate the effect of combined sur gery for complicated corneal penetrating trauma. Methods According to the degree of corneal penetrating trauma in 67 cases (67eyes),different combined operations were performed.Among them,15 eyes were undergone phacoemulsification and intraocular lens implantation,27 ey es were given posterior vitrectomy and extraction of intraocular foreignbodies,8 eyes were given reposition of retinal detachment with silicone oil filling,11 e yes were repaired by iridocele or coreoplasty,and 12 eyes with anterior and post erior capsular rupure and vitreous extrusion underwent anterior vitrectomy. Results Vision was <0.05 in 13 eyes (19.40),0.05～0.2 in 30 eyes (44.78%),0.3～0.4 in 18 eyes(26.87) and 0.5～1.0 in 6 eyes(9.0%) after the treat ment.The visual acuity of the 67 cases (67 eyes) improved,besides the shape of t he eyeball being well kept and the shape of pupil being recovered. Conclusion The combined surgery for complicated corneal penetrati ng trauma has been proved successful,producing good visual result and reduced th e course of disease with safety.
