盐酸氟西汀联合雌激素治疗绝经期抑郁症的临床观察

摘　　要：目的观察盐酸氟西汀联合利维爱用于绝经期抑郁症的疗效。方法将已绝经并符合抑郁症诊断的患者60例,随机分为两组,每组30例。联合治疗组用盐酸氟西汀和利维爱、对照组用利维爱进行治疗。治疗开始日及治疗第1月末和第2月末各随访1次,应用Zung氏抑郁自评量表(SDS)评价抑郁症程度,应用Kupperman绝经指数(KMI)评价绝经期症状。结果60例绝经期抑郁症患者中,轻度抑郁42例(70.0%),中度抑郁15例(25.0%),重度抑郁3例(5.0%)。开始治疗前,联合治疗组和对照组的SDS评分比较,差异无显著性(P>0.05)。治疗后第1月末和第2月末联合治疗组SDS评分分别低于对照组,差异非常显著(P<0.01)。开始治疗前,联合治疗组和对照组的KMI评分比较,差异无显著性(P>0.05)。治疗后第1月末和第2月末联合治疗组KMI评分分别低于对照组,差异非常显著(P<0.01)。联合治疗组的有效率96.66%,对照组的有效率46.66%,差异非常显著(P<0.01)。结论盐酸氟西汀联合利维爱对绝经期抑郁症的治疗效果较好,值得临床推广应用。Objective To observed the treatment effect of fluoxetine plus livial on menopausal depression.Methods Sixty patients with climacteric symptoms and depression were enrolled and randomly divided into two groups.One group was given fluoxetine plus livial and called combined treat group. The other group was given livial alone and called control group. All subjects were interviewed and evaluated with self-rating depression scale (SDS) and Kupperman menopause index (KMI) at months 0, 1 and 2 of the treatment.Results There were 42 cases of light depression, 15 cases of middle depression and 3 cases of severe depression in all. There was no difference between two groups in SDS scores and KMI scores before the trial (P>0.05). SDS scores at the 1st and 2nd months in combined treat group were significantly lower than that of control group respectively (P<0.01).KMI scores at the 1st and 2nd months in combined treat group were significantly lower than that of control group respectively (P< 0.01). The healing rate of combined treat group were 96.66%, higher than 46.66% of control group significantly (P<0.01).Conclusion The effect of fluoxetine plus livial on menopausal depression is obvious and can be used clinically.
