颈内静脉穿刺置管的解剖学基础及临床应用

摘　　要：在成人３０具尸体标本，结合颈内静脉穿刺置管术，观察了颈内静脉下１／３段的行程及其毗邻结构，并测量了颈内静脉的口径。根据解剖观察所得资料，对３０５例息者进行了颈内静脉穿刺置管。穿刺部位为胸锁关节上缘３～４ｃｍ，由胸锁乳突肌的锁骨头与胸骨头所形成陷凹处。插省长度右侧在１３．３～１４．３ｃｍ，左侧在１５．８～１６．８ｃｍ的范围内，以采用右侧插管为佳。临床实践证明效果满意。he course,syntopy and internal diameter of the lower one─third of internal jugular veinare observed and measured in 30 adult cadavers. The internal diameters of right and left internaljugular veins are respectively 15.6mm and 12.5mm. The lengths of right and left brachiocephalicveills and superior vena cava are 41.5mm,66.3mm and 65.0mm respectively. The surface projec-tion of the site for internal jugular vein intubation is located between the calvicular and sternalheads of sternocleidomastoid muscle,30 to 40mm superior to the sternoclavicular joint.Clinicalapplications in 305 cases of patients show that the proper lengths for right and left internal jugu-lar vein intubation are 133─143mm and 158─168mm respectively.Satisfactary results areachieved in all cases.
