急性脑梗死患者血清瘦素水平及与胰岛素抵抗相关性分析

摘　　要：目的探讨急性脑梗死患者血清瘦素浓度的影响因素及其与胰岛素抵抗的关系。方法用酶联免疫吸附法(ELISA)测定50例急性脑梗死患者和50例对照者的血清瘦素浓度。结果①女性肥胖者(BMI≥25)在急性脑梗死组比对照组血清瘦素明显增高,有极显著性差异(P=0.001)。②两组中女性血清瘦素水平均比男性增高(P=0.000);③血清瘦素水平与BMI呈显著正相关(P=0.000),与胰岛素敏感指数显著负相关(P=0.000),与FINS(P=0.000)和FBG(P=0.001)呈显著正相关。结论女性肥胖急性脑梗死患者的血清瘦素水平显著增高;血清瘦素水平与性别、BMI、胰岛素抵抗密切相关。Objectives To assay serum leptin levels in patients with acute cerebral infarction, and to analyze the correlation of serum leptin levels with sex, BMI, IR and other parameters. Methods The concentrations of serum leptin of 50 patients with acute cerebral infarction and 50 controls were assayed by ELISA. Body weight, height, length of waist and hip, blood pressure were compared in patients with acute cerebral infarction and control group. Plasma leptin, fasting plasma insulin (FINS) and fasting plasma glucose, TC, HDL-C, LDL-C, TG, CRP were also measured in patients with acute cerebral infarction. BMI, waist/hip ratio (WHR) and ISI were also calculated. Results ① Patients and controls were divided into four groups according to sex and BMI. ② Only the levels of serum leptin obese female patients were higher than that in obese female controls. ③ The serum leptin concentrations were highly correlated with sex, BMI (P = 0.000), ISI (P = 0.000), FINS (P = 0.000) and FBG (P = 0.001). Conclusions High serum leptin level is associated with acute cerebral infarction in obese female patients. The levels of serum leptin are correlated with sex,BMI and IR.
