摘  要:目的研究胸腔积液γ-干扰素(IFN-γ)水平对结核性与癌性胸腔积液鉴别诊断的价值。方法用酶联免疫吸附分析法(ELISA)和氨试剂法检测80例胸腔积液(40例结核性积液和40例癌性积液)中IFN-γ水平和腺苷脱氨酶活性,并将IFN-γ检测结果与ADA测定结果进行对比。结果IFN-γ对结核性积液诊断的敏感度为90%,特异度为100%,正确率为95%;ADA对结核性胸腔积液诊断的敏感度为80.0%,特异度为100%,正确率为90%。结论胸腔积液IFN-γ水平的检测对鉴别结核性与癌性胸腔积液明显优于ADA,可作为一种诊断结核性胸腔积液的有力手段。Objective: To study the value of interferon gamma (IFN-γ) in differentiating tuberculosis from malignant pleural effusions. Methods: Both IFN-γ and ADA of 80 specimens of pleural effusions (40 cases of tuberculosis pleural effusions and 40 cases of malignant pleural effusions) were analyzed by enzyme linked immunosorbet assay ( ELISA) and ammoniacal reagent method . Results: The sensitive rate and special rate of IFN-γ to diagnose tuberculosis pleural effusions were 90% and 100%,and that of ADA were 80% and 100%, the accuracy of IFN-γ and ADA were 95% and 90%. Conclusion: The results indicate that the detection of IFN-γ in pleural effusions is more sensitive, more accurate than ADA in differentiating tuberculosis from malignant pleural effusions and can be used as adjunctive measure in the diagnosis of tuberculosis pleural effusions.
