晚期消化系肿瘤腹腔内化疗作用的评价

摘　　要：目的:对照研究晚期消化系统恶性肿瘤的腹腔化疗和全身静脉化疗,观察并评价两种方法的近期疗效、远期疗效和毒副反应。方法:随机分为腹腔化疗组(腹腔组)45 例和静脉化疗组(对照组)31 例,腹腔化疗组中胃癌18例,大肠癌14 例,肝癌6例,其他癌症7 例。静脉化疗组中胃癌12例,大肠癌8例,肝癌5例,其他癌症6 例。记录观察近期疗效和毒副反应,随访患者并计算生存期。结果:近期疗效两组相仿。毒副反应中恶心呕吐、骨髓抑制症状,腹腔组明显低于对照组,而腹胀腹痛则腹腔组比对照组严重,肝功能损害两组差异无显著性。腹腔组中位生存期和1年生存率显著高于对照组,P<0 .05。结论:对晚期消化系统肿瘤患者进行腹腔内化疗,可提高生存质量,延长生存期。OBJECTIVE:To compare the effects and side effects of intraperitoneal chemotherapy and vein chemotherapy in the treatment of advanced gastrointestinal cancer.METHODS: A total of 76 patients were divided randomly into two groups. Forty-five patients were treated with intraperitoneal chemotherapy,including 18 cases of gastric cancer, 14 cases of colorectal cancer,6 cases of liver cancer and 7 cases of other cancers. Thirty-one patients were treated with vein chemotherapy, including 12 cases of gastric cancer,8 cases of colorectal cancer,5 cases of liver cancer and 6 cases of other cancers. All patients were followedup, and the near-term effect and side effect were investigated.RESULTS: The short effects of the two groups were similar.The rates of nausea/vomiting and myelosuppression were lower significantly in the intraperitoneal chemotherapy group than those in the vein chemotherapy,but the rates of abdominal pain and abdominal distension were higher in the former than those in the latter. There were similar rates of liver toxicity in the two groups. The rates of median survival and 1-year survival were higher significantly in the intraperitoneal chemotherapy group than those in the vein chemotherapy,P<0.05.CONCLUSION:Intraperitoneal chemotherapy can improve survival time and quality of life for the patients with advanced gastrointestinal cancer.
