摘 要:目的 检测上皮钙黏蛋白和α连接素在乳腺癌组织中的表达情况,分析它们与乳腺癌生物学特性的关系,探讨乳腺癌浸润、转移的机理。方法 采用免疫组织化学方法检测54 例乳腺癌及21 例癌旁正常组织(距肿瘤5 cm以上并经病理检查证实)中上皮钙黏蛋白和α连接素的表达情况。结果 21例癌旁正常组织中,上皮钙黏蛋白和α连接素均主要定位于腺泡或导管上皮细胞膜上,显示了细胞细胞间交界和细胞轮廓,二者均表达正常;54例乳腺癌组织中上皮钙黏蛋白和α连接素的异常表达率分别为51.9%和63.0%。上皮钙黏蛋白和α连接素的异常表达与组织学分级及高增殖活性密切相关。α连接素异常表达与TNM分期、腋窝淋巴结转移和术后远处转移显著相关。上皮钙黏蛋白异常表达与HER 2表达呈正相关。COX多元回归分析显示,两种蛋白表达均未能成为影响乳腺癌预后的独立指标。结论 乳腺癌中广泛存在上皮钙黏蛋白和α连接素异常表达,二者异常表达与乳腺癌的增殖、分化失控相关。Objective To detect expressions of E-cadherin and α-catenin in breast cancer and analyze the relationship between those expressions and biological behaviors of breast cancer. Methods Fifty-four female patients with breast cancer received modified radical mastectomy or radical mastectomy in our department from August 1998 to March 1999. Their ages ranged from 30 years to 76 years and the postoperative follow-up time ranged from 6 to 67 months. Sixteen patients died during their follow-up time. The expressions of E-cadherin and α-catenin in specimens of 54 breast carcinomas and 21 normal breast tissues around tumor were measured by immunohistochemical method. Results In normal breast tissues, E-cadherin and α-catenin were expressed on cell membrance of ductal and acinic cells. No abnormal expressions were found in normal breast tissues. The abnormal expression rates of E-cadherin and α-catenin in breast cancer were 51.9% and 63.0% respectively. Abnormal expressions of E-cadherin and α-catenin were significantly correlated with histological grade and proliferative grade. Abnormal expression of α-catenin was significantly correlated with TNM staging, axillary lymph node metastasis and postoperative distant metastasis. Abnormal expression of E-cadherin was positively correlated with expression of HER-2. COX multiple factor analysis suggested that neither E-cadherin nor α-catenin expression was an independent prognostic indicator of breast cancer. Conclusion Abnormal expressions of E-cadherin and α-catenin frequently occur in breast cancer. Abnormal expressions of E-cadherin and α-catenin are correlated with disturbance of proliferation and differentiation of breast cancer and its metastasis.
