摘  要:目的 构建孕兔妊娠肝内胆汁淤积症(ICP)模型,为ICP围产儿并发症发生机制的研究提供适宜的动物模型。方法 选择3 2只孕兔,随机分为4组。高、中、低剂量组自妊娠第2 2天,分别颈后皮下注射苯甲酸雌二醇0 2、0 1、0 0 5mg·kg-1·d-1,对照组颈后皮下注射生理盐水,直至分娩。妊娠第2 6天耳静脉采血检测血清生化指标,孕兔分娩时取部分肝脏组织观察病理变化。结果 ①与对照组相比,高、中剂量组孕兔血总胆汁酸(TBA)、天冬氨酸转氨酶(AST)、丙氨酸转氨酶(ALT)、甘胆酸(CG)均显著升高(P <0 0 1) ,总胆固醇(TCH )显著下降(P <0 0 1) ,而低剂量组仅TBA、CG较对照组有升高(P <0 0 5 ) ,其余生化指标统计学差异无显著性;②高、中、低剂量组均出现早产、胎死宫内,二者发生率明显高于对照组(P <0 0 5 ) ;③病理观察显示,高、中剂量组孕兔肝脏呈胆汁淤积改变。结论 皮下注射苯甲酸雌二醇可诱导妊娠晚期孕兔出现肝内胆汁淤积病理改变。Objective To establish a disease model for intrahepatic cholestasis in pregnant rabbits and provide a suitable animal model for the study on the mechanism of fetal complications of intrahepatic cholestasis of pregnancy (ICP). Methods Thirty-two pregnant rabbits were randomly divided into four groups. The high, moderate and low dosage groups were subcutaneously injected with benzestrofol at a dose of 0.2, 0.1 and 0.05 mg·kg -1 ·d -1 respectively and control group with 0.9% saline solution from day 22 of gestation to delivery. Blood tests for biochemical parameters were conducted on day 26. Specimens of livers were taken after cesarean for light microscopy. Results In high and moderate dosage groups, the serum levels of aspartate aminotransferase (AST), alanine aminotransferase (ALT), total bile acid (TBA) and glycocholic acid (CG) were significantly increased (P<0.01) and total cholesterol (TCH) were significantly reduced (P<0.01), as compared with control group. In low dosage group only TBA and CG were significantly increased (P<0.05), while other parameters remained unvaried. The rates of premature labor and fetal death in high, moderate and low dosage groups were significantly higher than those in control group (P<0.01). Obvious pathological changes of intrahepatic cholestasis were observed in the high and moderate dosage groups. Conclusion Benzestrofol can induce intrahepatic cholestasis in trimester pregnant rabbits.
