摘  要:目的 探讨缺氧诱导因子 1α(HIF 1α)、E 钙粘蛋白(E cadherin)在宫颈癌中表达的意义以及HIF 1α/E cadherin途径在宫颈癌发病过程中的调控作用。方法 取临床标本13 5例,采用免疫组织化学方法检测HIF 1α、E cadherin蛋白的表达。结果 正常宫颈组织中几乎没有HIF 1α的表达,宫颈上皮内瘤变组织中HIF 1α阳性表达率为12 5 % ,宫颈癌组织从0期到Ⅳ期HIF 1α表达阳性率不断升高,在不同组织学分级(即高、中、低分化)的宫颈浸润癌组织中,HIF 1α的阳性表达与组织学分化程度呈负相关(r =-0 769P <0 0 1) ;在不同临床及病理分期中HIF 1α的表达水平与E cadherin呈负相关。而随着从正常宫颈→宫颈上皮内瘤变→原位癌→浸润癌的转化,E cadherin的阳性表达率呈递减趋势,在不同的临床分期中E cadherin的阳性表达率逐渐降低,但与组织学分化程度呈负相关(r =-0 872 ,P <0 0 1)。结论 低氧条件下HIF 1α通过对其下游因子E cadherin表达水平的调控作用,参与宫颈癌的侵袭、转移,这在宫颈癌的发生、发展过程中可能起重要的作用。两者联合检测可以更准确地判断宫颈癌的恶性程度及生物学行为。Objective To study the significance of the expressions of HIF-1α and E-cadherin in cervical carcinoma. Methods The expressions of HIF-1α and E-cadherin (E-cad) were determined in the clinical specimens of 109 cases of cervical carcinoma, 16 cases of cervical intraepithelial neoplasm and 10 cases of normal cervical tissue with immunohistochemical assay. Results There was nearly no expression of HIF-1α in the normal cervical tissue. The positive expression of HIF-1α and E-cad were 81.4% (70/86) and 28.6% (25/86) respectively in the tissue of invasive cervical carcinoma. The rate of positive expression of HIF-1α was significantly higher in invasive cervical carcinoma than in normal cervical tissue and cervical intraepithelial neoplasm (P<0.01). The positive rate of E-cad was significantly lower in cervical carcinoma in situ and invasive cervical carcinoma than in normal cervical tissue and in cervical intraepithelial neoplasm (P<0.01). There was a significant correlation of the expression rate of HIF-1α and E-cad with the FIGO stages or histological subtypes of cervical carcinoma and a significant negative correlation of the expression of HIF-1α and E-cad. Conclusion HIF-1α is over expressed in cervical carcinoma and may induce the expression of E-cad. Both HIF-1α and E-cad may exert important effects of the invasiveness of the carcinoma. The expression of HIF-1α and E-cad can serve as an indication of the metastasis of cervical carcinoma.
