表阿霉素单次或多次膀胱灌注预防初发性膀胱癌与复发性膀胱癌术后复发的疗效比较

摘　　要：目的:探讨单次或多次表阿霉素膀胱灌注对预防初发性膀胱癌与复发性膀胱癌术后复发的疗效。方法:将47例初发性膀胱癌术后患者分为A(23例)和B(24例)二组,同时将52例复发性膀胱癌术后患者分为C(26例)和D(26例)二组。A、C组在术后6h内表阿霉素80mg/次单次灌注;B、D组在术后1周内表阿霉素50mg/次膀胱灌注,每周1次,共4次,后每月1次,共1年。比较各组术后2年肿瘤复发率及毒副反应发生情况。结果:2年复发率:C组复发率高于A、B、D组,A、C组药物毒副反应发生率低于B、D组。结论:表阿霉素单次膀胱灌注预防初发性浅表性膀胱癌术后复发的作用可靠,毒副反应发生率低;对复发性膀胱癌术后复发的预防,多次膀胱灌注比单次膀胱灌注疗效可靠。Objective: To explore the clinical therapeutic effect on the master single test or multiple irrigation of bladder with epirubicin to prevent first and recrudescent bladder carcinoma after operation . Methods: 47 patients with first bladder carcinoma after operation(or surgery) were divided two groups: the A group ( 23 cases) and B group (24 cases) , and 52 patients with recrudescent bladder carcinoma after operation were divided two groups: the C group ( 26cases) and D group (26 cases) , the patients in groups A and C received a single-dose instillation of 80 mg epirubicin immediately after operation within 6 hours. Those in groups B and D received 50 mg epirubicin after operation within 1 week , the same dose was given to B group and D group weekly for 3 times , and 50 mg monthly for 12 months , to compare the relapse rate of tumor within 2 years and incidence rate of side reaction . Result:The relapse rate of tumor in C group was significant higher than that in A group, B group and D group within 2 years, The incidence rate of side reaction in A group and C group was significant lower than that in B group and D group . Conclusions :There was significant clinical therapeutic effect on the master single test irrigation of bladder with epirubicin to prevent first (primary) superficial bladder carcinoma after operation with lower incidence rate of side reaction . and the clinical therapeutic effect on the multiple irrigation of bladder with epirubicin to prevent recrudescent bladder carcinoma after operation was more significant compared with master single test irrigation of bladder with epirubicin.
