24h动态脑电图在儿童癫痫和非癫痫疾病的应用价值

摘　　要：目的　探讨2 4h动态脑电图在儿童癫痫的诊断与鉴别诊断中的应用价值。方法　2 0 0 2年2月至2 0 0 4年10月在我院儿科病房和门诊就诊的具有发作性症状病例14 9例,其中拟诊癫痫5 2例,非癫痫性发作性疾病97例,全部病例作常规脑电图(EEG)和2 4h动态脑电图(AEEG)检查。结果　拟诊癫痫5 2例,经2 4h动态脑电图监测,结合其临床表现,确诊为癫痫4 0例;而非癫痫性发作性疾病97例,经2 4h动态脑电图监测,并结合其临床表现,90例被除外癫痫。结论　2 4h动态脑电图对儿童发作性疾病,尤其是癫痫与非癫痫性发作性疾病的诊断与鉴别诊断具有重要价值,同时2 4h动态脑电图还可揭示癫痫灶的起源,有助于癫痫和癫痫综合征的分类,为正确治疗提供依据。Objective:To evaluate the significance of 24-hour AEEG in diagnosis and distinguishing diagnosis of epilepsy in childhood. Methods: February 2002～Oct 2004,among 149 cases of patients with seizuring symptoms both from our pediatric admission Dept(department) and out-patient dept, 52 cases were preliminarily diagnosed as epileptic seizures,97 cases were non-epilepsy seizures.All the cases had been under monitoring of both by routine EEG and 24-hour AEEG. Results: 40 of 52 children whose episodes had been preliminarily diagnosed as epileptic seizures were identified as epilepsy by 24-hour AEEG and clinic manifestation,and the other 97 cases of non-epilepsy seizures,92.8 percent of which were excluded from epilepsy, Conclusion: 24-hour AEEG have great significance on the diagnosis and distinguishing diagnosis of seizuring diseases in childhood ,especially for epileptic seizures and non-epilepsy seizures,meanwhile,24-hour AEEG can disclose the sources of epileptic focuses,and was helpful to classify epilepsy and epileptic syndromes,and also provide correct therapeutical information.
