摘  要:目的:探讨中枢神经系统疾病患者脑脊液中肌酸磷酸激酶、乳酸脱氢酶、天冬氨酸氨基转移酶、α-羟丁酸脱氢酶和丙氨酸氨基转移酶活性变化对疾病评估的价值。方法:选择2001-03/2003-05在西京医院住院的临床确诊患者107例,其中化脓性脑膜炎18例,结核性脑膜炎16例,病毒性脑膜炎15例,脑血管疾病20例,急性颅脑损伤20例,肿瘤18例。对照组选择20例临床无器质性病变的同期住院患者,脑脊液常规、生化均正常。所有被试者检测脑脊液,离心取上清当日测定,采用酶法测定肌酸磷酸激酶,乳酸脱氢酶,天冬氨酸氨基转移酶,α-羟丁酸脱氢酶,丙氨酸氨基转移酶。结果:按意向处理分析,127例均进入结果分析。①脑脊液肌酸磷酸激酶:急性颅脑损伤最高,高于其他组犤(574±103)nkat/L,P均<0.01犦;以结核性脑膜炎为最低,与对照组相比无显著性差异犤(200±43),(183±38),P>0.05犦。②脑脊液乳酸脱氢酶,α-羟丁酸脱氢酶活性:以急性颅脑损伤升高最显著,与其他组相比均有非常显著性差异(P<0.01);脑血管疾病与结核性脑膜炎虽然均高于对照组,但均差异不显著(P>0.05)。③脑脊液天冬氨酸氨基转移酶,丙氨酸氨基转移酶:在几组中枢神经系统疾病中活性均增高,以化脓性脑膜炎、脑血管疾病、急性颅脑损伤、病毒性脑膜炎时增高显著,结核性脑膜炎最低,但?AIM: To study the value of the activities of creatine phosphokinase, lactic dehydrogenase, aspartate aminotransferase, α hydroxdutyric dehydrogenase and alanine aminotransferase in cerebrospinal fluid in th evaluation of the injury degree of patients with central nervous system disease. METHODS: A total of 107 in patients with acute bacterial meningitis(n=18), tubercular meningitis(n=16), viral meningitis(n=15), cerebrovascular disease(n=20), acute head injury(n=20) or tumor (n=18) were selected from Xijing Hospital from March 2001 to May 2003. Another 20 patients without organic lesion and with normal cerebrospinal fluid and biochemistriy levels admitted during the same period were selected as control group. Cerebrospinal fluid was measured and centrifuged to get supernate which was measured within 24 hours. Enzyme method was used to measure the activities of creatine phosphokinase, lactic dehydrogenase, aspartate aminotransferase, α hydroxdutyric dehydrogenase and alanine aminotransferase.RESULTS: According to intention to treat, all 127 participants were involved in the result anaysis.① The activity of creatine phosphokinase in cerebrospinal fluid was the highest in acute head injury group as compared with other groups [(574± 103) nkat/L, P< 0.01],and was the lowest in tubercular meningitis group, which did not significantly differ that in control group [(200± 43), (183± 38) nkat/L, P >0.05]. ② The activity of lactic dehydrogenase and α hydroxdutyric dehydrogenase increased significantly in acute head injury group, significantly different from other groups(P< 0.01),and it was higher in cerebrovascular disease and tubercular meningitis groups than in control group, but there was no significant difference(P > 0.05). ③ The activity of aspartate aminotransferase and alanine aminotransferase increased significantly in groups of central nervous system disease as compared with that in control group (P< 0.05),and was the highest in acute bacterial meningitis, cerebrovascular disease, acute brain injury, vi
