大脑及脊髓原始神经外胚叶肿瘤的临床和病理及免疫组化相关性研究

摘　　要：目的:探讨大脑及脊髓的原始神经外胚叶肿瘤(PNET)的临床、病理及免疫组化之间的相关性。方法:收集本院大脑及脊髓PNET18例,采用Spearman相关分析方法,对患者年龄,菊形团、栅栏状坏死、核分裂相和CD99、神经胶质原纤维酸性蛋白(GFAP)、突触素(Syn)、神经特异性烯醇化酶(NSE)免疫组化的半定量指标,共八项指标之间均进行相关性分析。结果:免疫组化CD99、Syn和NSE大部分阳性,其中CD99阳性率最高,GFAP也大部分阳性,与上述三项大部分重叠。此外,9例患者的肿瘤同时具有菊形团和胶质细胞分化。相关分析的结果:患者年龄与栅栏状坏死的数量、栅栏状坏死的数量与GFAP、NSE与Syn均呈正相关(P<0.05);菊形团及核分裂相数量分别与其他各项指标之间均无显著相关性。结论:大多数PNET均有神经元、胶质细胞双潜能分化。Objective: To analyze the relationship among clinical , pathological and immunohistological profiles of primitive neuroectodermal tumors(PNET) of cerebrum and spinal cord. Methods: In 18 cases of patients with PNET, semiquantitative correlation analysis was used on the age of patients, rosettes, necrosis, mitotic activity, CD99, synaptophysin(Syn) immunoreactivity, glial fibrillary acidic protein(GFAP) and NSE. Results: CD99, synaptophysin(Syn) and NSE immunoreactivity were almost positive, and so was GFAP. 9 cases of patients with PNET showed Homer-Wright type rosettes and glial differentiation simultaneously. Semiquantitative correlation analysis showed that: ①the age of patients was positively correlated with necrosis(P<0.05); ②the number of necrosis was positively correlated with GFAP(P<0.05); ③NSE was positively correlated with Syn(P<0.05); ④the number of necrosis and mitotic activity was not correlated with other parameters, respectively. Conclusion: Most of PNET had both glial differentiation and neuro- differentiation.
