低剂量5-FU联合化疗治疗老年晚期消化道腺癌的近期疗效观察

摘　　要：目的观察低剂量5-氟尿嘧啶(5-FU)联合顺铂(DDP)、四氢叶酸钙(CF)和羟基喜树碱(HCPT)方案治疗老年晚期消化道腺癌的疗效。方法第1通道5-FU188mg(m2·d)持续静脉输注,每天不少于18h,每周5天。其他药物经第2通道静脉输注DDP5mg(m2·d),1h,每周5天;CF60mg(m2·次),1h,隔日1次,每周3次;HCPT3.3mg(m2·次),1h,隔日1次,每周3次。连用3周休息1周为1个疗程,以上方案连用2个疗程后评价疗效。结果21例中PR15例,CR0例,NC4例,PD2例,RR为71.4%;未见Ⅲ～Ⅳ度的不良反应。结论低剂量5-FU持续输注联合低剂量DDP、CF、HCPT方案治疗老年晚期消化道腺癌是高效低毒的化疗方案。Objective To investigate the effects on the aged patients with terminal alimentary tract adenocarcinoma treated by the chemotherapy scheme consisting of low-dose 5-FU,DDP,CF and HCPT. Methods 5-FU 188 mg/(m2·d) was continuously infused through one vein channel for at least 18 hours a day, 5 days a week. Other drugs were infused through the other vein channel: DDP 14.5 mg/(m2·d) iv in 1 hour, 5 days a week; CF 60 mg/(m2·d) and HCPT 3.3 mg/(m2·d) iv in 1 hour respectively, q2d, 3 times a week. Three sequential weeks of treatment and one week of break was one course of treatment, the therapeutic effect was evaluated after 2 courses. Results In the 21 patients, there were 15 cases of PR, no CR, 4 NC and 2 PD, with the RR being 71.4%. No Ⅲ～Ⅳ degree side effects were encountered. The life quality of the patients were obviously improved. Conclusion To treat the terminal alimentary tract adenocarcinoma in aged patients, the chemotherapy scheme composed of protracted infusional low-dose 5-FU and low-dose DDP, CF and HCPT is of high efficiency and low toxicity.
