实时RT-PCR及电化学技术检测胃癌术中腹腔灌洗液中游离癌细胞的比较

摘　　要：目的建立实时RT-PCR及电化学技术两种较敏感的检测胃癌术中腹腔灌洗液中游离癌细胞的方法,并对这两种检测方法进行比较。方法分别采用实时荧光RT-PCR和电化学发光方法检测65例胃癌术中腹腔灌洗液中CEAmRNA及CEA蛋白,同时进行腹腔冲洗细胞学方法(peritoneallavagecytology,PLC)涂片寻找癌细胞。收集5例良性胃疾病的术中腹腔灌洗液,5例健康志愿者血液为阴性对照。结果(1)RT-PCR方法检测游离癌细胞的阳性率为47.7%,电化学发光法的阳性检出率为35.4%,两者均无假阳性。而PLC的检出率仅12.3%。(2)电化学发光方法具有快速、试剂稳定、无污染和衰变、自动化程度高,且较经济等优点。(3)CEAmRNA、CEA蛋白阳性率在不同的病理分期组(+期和+期)中差异有显著性(P<0.05)。结论实时荧光RT-PCR方法和电化学发光方法检测游离癌细胞各有优势,游离癌细胞的存在与肿瘤病理状态有一定相关性。Objective To evaluate real-time RT-PCRand electrochemistry immunonassay in detection of free cancer cells in peritoneal washes of gastric cancer patients during surgery. Methods The CEAmRNA in peritoneal washes in 65 cases of gastric cancer was detected by real-time RT-PCR and electrochemistry immunonassay. Peritoneal lavage cytology (PLC)was applied simultaneously to detect the free cancer cells. Negative controls included the peritoneal washes form 5 cases of benign gastric disease and the blood samples from 5 adult healthy volunteers. Results Positive rate of free cancer cells by real-time RT-PCR was 47.7%, by electrochemistry immunonassay was 35.4%, while by PLC it was only 12.3%. The positive rates of CEA mRNA and CEA protein were significantly different between different pathologic classification groups(Ⅰ+Ⅱ and Ⅲ+Ⅳ stage). Conclusion Real-time RT-PCR and electrochemistry immunonassay have advantage to detect free cancer cells in peritoneal washes and free cancer cells in peritoneal washes are related to tumor pathologic stages.
