摘 要:目的分析治疗眼部真菌感染7种药物的有效性。设计对照实验研究。研究对象眼部分离76株真菌及7种抗真菌药物。方法用微量液基稀释法(M38P)测定特比萘芬(TBN)、两性霉素B(AmB)、酮康唑(KCZ)、咪康唑(MCZ)、伊曲康唑(ICZ)、氟康唑(FCZ)和5氟胞嘧啶(5FC)7种抗真菌药对眼部分离真菌的最低抑菌浓度(MIC)范围和平均值。主要指标药物最低抑菌浓度。结果TBN对眼部真菌作用最强,对镰刀菌和曲霉菌的MIC值和平均值最小,茄病镰刀菌的MIC为0.15~8μg/ml,平均值为1.10μg/ml。AmB对茄病镰刀菌作用仅次于TBN,其MIC为2.00~8.00μg/ml,平均值为3.97μg/ml。KCI、MCA和ICZ对茄病镰刀菌作用稍差,对其它菌种均有一定作用。5FC和FCZ(除黑曲霉和串珠镰刀各1例)最低抑菌浓度均>64μg/ml的最高浓度。结论TBNF对眼部真菌作用最强,AmB次之,KCZ、MCZ和ICZ对不同菌属均有作用,FCZ和5FC的作用较差。Objective To explore the effect of seven kinds of antifungal drugs to fungi isolated from keratomycotic.Design Comparative, experimental study.Participants Fungi isolated from keratomycotic and seven kinds of antifungal drugs.Methods The minimum inhibitory concentration (MIC) range and mean of terbinafine (TBN), amphotericin B(AmB), ketoconazole (KCZ), miconazole (MCZ), itraconazole (ICZ), fluconozole (FCZ), and 5-flucytosin (5-FC) against fungi were determined with broth microdilution of M38-p recommended by NCCLS.Main Outcome Measures The minimum inhibitory concentration of antifungal drugs.Results TBN showed good activity, MIC range and mean of TBN to fusarium and aspergillus were respectively lowest.The antifungal activity of AmB was lower than that of TBN and higher than other drugs.5-FC and FCZ had poor inhibitory activity (MIC>64μgml).Conclusion The effect of TBN to fungi isolated from keratomycotic is very strong and that of AmB is next to it.KCZ, MCZ and ICZ show different antifungal activity, 5-FC and FCZ have poor inhibitory activity.
