原发性肝外胆管癌的影像学表现（附90例报告）

摘　　要：笔者报告了经手术、病理证实的９０例肝外胆管癌的影像学表现。细胞学诊断为腺癌和鳞癌。病变位于肝外胆管上段６７例（７４．４％），中段４例（４．４％），下段１０例（１１．１％）和广泛型９例（１０．０％）。直接胆道造影的特征有阻塞、狭窄和息肉三型。笔者讨论了胆管癌在恶性阻塞性黄疸发病中的地位和影像学对发现胆管癌的作用，并提出Ｂ超、ＰＴＣ为本病的最佳影像学方法。The imaging features of primary extrahepatic bile duct carcimona in 90 cases with sur-gical and patholpgic comfirmation were reported.The cytologic diagnosis of the disease included adeno-carcinoma and squamous cell carcinoma.The lesion involved the upper part of the bile duct in 67 cases(74.4%),the middle part in 4(4.4%),the lower part in l0 (11.1%)and diffuse type in 9 (10.0%).The findings in direct chola ngiography could be classified as three types:obstruction,stenosis andpolyp.The malignant obstructive jaudice,and the imaging methods for the diagnosis were discussed.Itwas suggested that US and PTC were the modalities of choice for diagnosis of this disease.
