乳癌中突变P53和nm23-H1基因的表达及临床意义

摘　　要：目的　探讨乳癌中突变Ｐ５３ 和ｎｍ２３ － Ｈ１ 基因的表达与临床因素及淋巴结转移、远处转移和复发之间的关系；方法　应用免疫组化ＬＳＡＢ法研究乳癌的Ｐ５３ 、ｎｍ２３ － Ｈ１ 基因表达；结果　ｎｍ２３ － Ｈ１ 低表达与淋巴结及远处转移相关（ Ｐ＜ ０ ．０１ ，Ｐ＜ ０．０１） ，ｎｍ２３ － Ｈ１ 基因在低分化乳癌中表达显著降低，与中、高分化乳癌相比均有统计意义，突变Ｐ５３ 基因表达与淋巴结转移和远处转移相关（ Ｐ＜ ０ ．０２ ，Ｐ＜ ０ ．０５） ，与肿瘤大小相关，与组织学分级相关（ Ｐ＜ ０ ．０１ ，列联系数Ｐ＝ ０ ．３２） ，两者的表达与其它病理因素无关，两者联合表达中，Ｐ５３低表达和ｎｍ２３ － Ｈ１ 过表达组淋巴结转移率较低，与其它组相比均有统计意义，突变Ｐ５３ 过表达和ｎｍ２３ － Ｈ１低表达在总病例中远处转移率最高，与其它组相比均有统计意义；结论　认为二种基因表达对淋巴结和远处转移均起重要作用， 检测它们在乳癌中的表达，对指导临床治疗、随访、评价乳癌患者的预后有重要的参考意义。Objctive To study the correlation between mutant P53 and nm23-H1 gene products expression and its clinical factors,lymph node metastasis,distant metastasis and relapse.Methods P53 and nm23-H1 gene products were studied in 90 breast carcinomas using moloclonal antibody.Results Low expression of nm23-H1 gene correlated with reginal lymph node and distant matastsis(P<0.01,P<0.01 respectively) . Expression of nm23-H1 in Bloom and Richardson( BAR) grade 3 was significantly lower than in grade 1 and grade 2.Expression of mutant P53 gene correlated with tumor size(P<0.05)and BAR grade(P<0.01,P= 0. 32) , lymph node and distant metastasis( P< 0. 02, P< 0. 05 respectively) . No relationship were found between P53, nm23- H1 and age,menstrual status,venous invasion,clnical TNM stage. Lymph node metastasis rate was the lowest in co-expression group of P53 gene low expression and nm23- H1 gene over expression,significantly lowe than other groups. Distant metastasis rate was higher in co-expression group of P53 gene over expression and nm23-H1 gene low expression. Conlusion Our data indicated that nm23-H1,P53 play an important role in lymph node and distant metastasis in breast cancer patient, and have important significance in assessing prognosis,following up and guiding clinical therapy.
