摘  要:利用B超直方图检测99例健康老年人、99例老年高脂血症患者及45例脂肪肝患者的肝脏,同时与84例健康非老年人作对比观察。发现直方图距零位点,前三组近区依次逐渐右移、远区左移;谱宽近区,老年组较非老年组稍小,高脂组、脂肪肝组依次增大,远区三组依次缩小;平均灰阶(AV),前三组近区依次升高,远区依次递减;每组近区与远区平均灰阶之差(AV差)依次增大。AV差表明声波衰减有重要的意义。健康老年人AV差<5者占92.9%,颇接近非老年组;老年高脂血症患者AV差5~8者占72.7%;老年脂肪肝患者AV差>8者为100.0%。初步认为AV差可作为肝脏B超直方图的定量诊断指标。Abstract By using histogram patterns of hepatic B-mode ultrasonography 99 healthy old subjects,99 old patients with hyperlipidemia,45 patients with fatty liver and 84 healthy young subjects were studied.It was found that,from histogram pattern to the zero point,the near zones were moved to the right and the distant zones to the left in the first three groups.The curve width of near zones was slightly shorter in the aged than that in the young,while it was lengthened successively in the groups of hyperlipidemia and fatty liver.The distant zones were reduced successively in the first three groups.In the first three groups the average gray scale (AV) in the near zones gradually increased,while in distant zones it decreased.The differences between the AV in the near and distant zones were increased in the successive groups.AV differences indicate wave attenuation that may delineate the fatty nature of the liver.The rate of AV differences of less than 5 in the healthy aged was 92.9%;this was close to that in the controls.The rate of AV differences between 5-8was 72.7% in the group of hyperlipidemia.AV diffences were all greater than 8 in the group of fatty liver.Consequently,we are of the opinion that AV difference is a quantitative criterion of hepatic histogram pattern to delineate the fatty nature of the liver.
