老年性垂体腺瘤的外科治疗

摘　　要：本文报告老年性垂体腺瘤２６例。临床上主要表现为视功能障碍（８８％）和垂体功能低下（６９％）。来院同时伴有其它系统疾病的２１例（８１％），其中高血压８例（１例ＡＣＴＨ腺瘤除外），糖尿病４例，慢性肺炎肺气肿３例，心脏病和肾功能不全者各３例。本文对经额、经蝶窦入路切除垂体腺瘤的疗效进行了比较，认为对于老年垂体腺瘤患者经蝶窦显微外科垂体腺瘤切除术疗效好，并发症少，是一种较为安全有效的治疗方法。Abstract Twenty-six patients of senile pituitary adenoma were operated by transfrontal or transsphenoidal procedure in our department during Jan. 1963～Jan. 1993. The age of patients ranged from 60 to 74 years with an average of 63. 2 years. Clinical manifestations are mainly hypopsia and /or visual field defect(88%) and hypopituitarism (69% ). Twentyone patients combined with diseases of other system (81%) , e. g. hypertension, diabetes mellitus, chronic pneumonectasis, cardiac disease and renal insufficiency. The result of our operative experience suggest that transsphenoidal procedure is more effective and less operative complications. and is much better then transfrontal procedure for treatment of senile pituitary adenoma.
