流式细胞光度术在膀胱肿瘤诊治中的意义

摘　　要：应用流式细胞光度术对５２例膀胱肿瘤１３９次膀胱冲洗液中瘤细胞的ＤＮＡ含量进行了检测。检查结果与细胞学及病理分级进行了比较。结果表明：细胞学检查的准确性明显低于流式细胞ＤＮＡ含量的检查。肿瘤病理分级越高，浸润越深，其瘤细胞内ＤＮＡ异倍体含量越高。对于高异倍体膀胱肿瘤需行根治性膀胱全切；单纯电切或膀胱部分切除效果较差，既使术后应用膀胱内灌注化疗也有很高的复发率。流式细胞检测与膀胱肿瘤病理分级的相关性好，特别是在高分级肿瘤，但不能替代膀胱镜检查。AbstractThe cellular DNA contents of 139 bladder irriga-tions from 52 cases of bladder tumors were determinedby flow cytometry. The results were compared withthose of pathological grading and exfoliative cytology.It is suggested that cytoIogical results were less accu-rate than DNA contents. High aneuploid DNA con-tents in tumor cells were marked by low grade anddeep penetration of the bladder tumor. Total cystecto-my was needed for bladder tumor with high aneuploidDNA,and partial cvstectomv and TURBT for that tu-mor showed high recurrence with instillation of BCG.The results showed that DNA content of tumor is cor-related with the nuclear grade , especially in poorly dif-ferentiated neoplasms,but fIow cvtometrv can not re-place cystOscopy eXamInatlon.
