显微外科－超声吸引手术刀切除海绵窦肿瘤

摘　　要：采用显微外科－超声吸引手术刀切除６例海绵窦（Ｃａｖｅｒｎｏｕｓｓｉｎｕｓ，ＣＳ）内肿瘤，５例经ＣＳ外侧壁入路，１例经上壁入路，其中全切除５例，次全切除１例，无手术死亡。该手术应根据肿瘤的具体情况，选用手术进路，应用显微外科－超声吸引手术刀（ＣＵＳＡ）切除肿瘤。头抬高２０～３０°有助于ＣＳ内填塞止血，对术中可能伤及颈内动脉（ＩｎｔｅｒｎａｌｃａｒｏｔｉｄＡｒｔｅｒｙ，ＩＣＡ）者，应首先显露颈部ＩＣＡ及其床突上部，以备必要时可暂时孤立ＩＣＡ，并予以直接修复或重建。AbstractCUSA-microsurgical removal of 6 casesof tumors invofving cavernous sinus(CS )were reported. Five patients were operatedon via the lateral wall of the CS,One viathe superior wall. There was not surgical mor-tality. Total turmor removal was achieved in5 cases, subtotal in 1 case. The authors empha-sized that the different approach es shouldbe used a ccording to the locations of the tu-mot.CUSA-microsurgical technique might beuseful for removal of tumors. Head rised 20-3 0 degrees facilitates the control of thevenous hemorrhage of CS , the internalcarotidartery(ICA)in the neck and at supercli-noid segment should be exposed in thosewhich may be injured, so that the ICA canbeisolated transiently and repaired or recons-tructed directly in case it happens.
