自体骨髓移植治疗白血病59例临床分析

摘　　要：用自体骨髓移植（ＡＢＭＴ）治疗５９例急性非淋巴细胞白血病（ＡＮＬＬ）和急性淋巴细胞白血病（ＡＬＬ），单次移植４５例，双次移植１４例。结果单次ＡＢＭＴ者缓解时间１￣＋～６０￣＋月，中位数１１月，复发者１５例，占３３．３％。１年、３年和５年预期缓解率分别为６７．６±７．７％，５４．２±１０．８％和３８．７＋１５．２％，预期生存率分别为８８．９±５．１％，６６．７±８．１％和３３．５±１５．１％。１４例双次移植者缓解时间１１￣＋～６１￣＋月，中位数２８月，明显高于单次移植者（Ｐ＜０．０５）。其中１年、３年和５年预期缓解率分别为１００％，７３．３±１６．２％和７３．３±１６．２％，生存率分别为１００％，８９．５±１０．０％和７５．７±１５．２％。分析影响移植后缓解时间的因素，发现年龄，缓解至移植时间等对缓解期均无明显影响。单用化疗作为预处理方案者，效果明显优于化疗及放疗合用者。AbstractFifty-nine patients (39 males and 20 femals) with a-cute lcukemias including 44 non-lymphocytic and 15lymphocytic leukemias were adopted autologous bonemarrow transplantation (ABMT). The transplantationswere carried out within 2 to 55 months after the first re-mission except 7 after the second remission. Out of the59 patients, 14 underwent double ABMT. The resultsof the singleABMT group showed that the remissiondutations were 1 to 60 months (mean 22 months).There were 15 cases (33. 3% ) relapsed during the ob-serving period. Survival time was 5 to 100 monthsfmean 22 months). The predicted first, third and fifthyear's remission probability was 67. 6± 7. 79% , 54. 2 ±10. 8 % and 38. 7±15. 2 %, the predicted survival prob-ability was 88. 9 ±5. 1 % , 66. 7 ± 8.1 % and 33. 5±1 5.1% respectively. For the 14 double-ABMT cases, theremission duration was 11 to 61 months (mean 28months) and survival tiine was 10 to 68 months (mean34 months) , both being strikingly longer than that forthe single-ABMT (P<0. 05). Its predicted first , third ,fifth year's remission and survival probability was100% , 73. 7±16. 2% ,73.3±16. 2% and 100% , 89. 5±10. 0% , 75. 7±15. 2 % respectively. Varietes analysisshowed that the effects of patient's age and the intervalbetween remission and transplantation on the remissionduration were not significant.
