摘  要:目的比较单次屏气2D-thick-TS(E厚层)和2D-thin-HAST(薄层)磁共振胰胆管造影(MRCP)技术的临床诊断价值。方法对231例疑有胆胰疾病的患者分别行厚层和薄层MRCP技术成像,分析、对比影像学表现,并与手术、逆行胰胆管造影(ERCP)、B超结果进行比较。结果厚层和薄层MRCP对单一胰胆管结构的显示无明显差异(P>0.05)。但对胰胆管全貌(包括正常和病变时)的显示,厚层MRCP优于薄层MRCP,且具有明显的差异(P<0.01)。对肝内胆管结石和胆总管结石,薄层MRCP较厚层MRCP显示率均高(P分别<0.01和<0.05);对其他胰胆管疾病的显示,二者均无统计学意义(P>0.05)。结论厚层和薄层MRCP成像均能很好地显示胰胆管结构与病变,但厚层MRCP对胰胆管的全貌显示有优势,而薄层MRCP对病变的细微结构的显示,特别是细小结石的诊断更具优势。Objective To compare single-breath-holding 2D-thick-TSE with 2D-thin-HASTE MR cholangiopancreatography(MRCP) in clinical effectiveness. Methods 231patients with doubtful pancreatic and bile diseases underwent 2D-thick-TSE and 2D-thin-HASTE MRCP. Imaging features were analyzed and compared with that of ERCPand B-type ultrasound. Results Thick MRCP was not significantly different fromthin MRCP in displaying structure of individual pancreatic and bile(P > 0.05), butthick MRCP was significantly superior for the overall pancreatic and biliaryimaging(P < 0.01). Thin MRCP was significantly superior for diagnosing smaller leasis(P < 0.01 and stone(P < 0.05). As for usefulness of displaying other pancreatic and biletract diseases, both ERCPs were not significant (P > 0.05). Conclusion Both thickand thin MRCPs are able to display structure and lesions of pancreatic and bile tract.The thick is superior for the whole biliary imaging and the thin is good at displaying theoverall figure. Thin MRCP is superior for detecting fine structures, especially for fineleasis and stone.[
