白蛋白、球蛋白、白/球比值与肝组织病理的关系

摘　　要：目的:探讨白蛋白(ALB)、球蛋白(GLB)、白/球比值(A/G)与慢性肝炎肝组织炎症活动度及纤维化程度的相关性,并探讨它们在慢性肝炎分度中的诊断意义。方法:295例慢性肝炎患者经肝脏活检行常规病理检查,同时肝活检前一周内采血检测血清白蛋白、球蛋白,计算白/球比值,探讨它们与肝脏病理的关系。结果:血清ALB、GLB及A/G与肝组织病理分级及纤维化分期均有相关性(r分别为-0.395、0.244、-0.435及-0.332、0.223、-0.305;P均小于0.0001),ALB、A/G水平随炎症活动度(G1→G4)及纤维化分期(S1→S4)而依次降低,各期中差异有显著性;GLB随炎症活动度(G1→G4)及纤维化分期(S1→S4)而依次升高,各期中差异有显著性。在慢性肝炎的轻度、中度至重度,A/G值下降较血清ALB下降及GLB升高的百分率均更高,差异有非常显著性意义,P<0.01。结论:白蛋白、球蛋白、白/球比值对判定肝脏病理程度有一定的参考价值,A/G值能更准确地反映肝脏病理程度。Objective:To investigate the relationship between ALB?GLB?A/G and liver histopathologic changes in chronic hepatitis,also to discuss their significance on diagnosing chronic hepatitis grade.To compare these levels with the histopathological grading and staging.Methods:Performliver biopsies and regular pathologic examination for295chronic hepatitis patients,the serum markers of ALB?GLB?A/G are measured in one week before liver biopsies,In order to compare these levels of the markers with the histopathological grade and stage of the patients.Results:The serum levels of ALB?GLB?A/G are closely correlated with the histopathological grade and fibrosis stage(r=-0.395?0.244?-0.435and-0.332?0.223?-0.305;respectively;P<0.0001).The levels of ALB?A/G reduce obviously with the histopathological grading(G 1 →G 4 )and fibrosis staging(S 1 →S 4 );GLB raises obviously with the histopathological grading(G 1 →G 4 )and fibrosis staging(S 1 →S 4 );The percentage of decrease of the level of A/G was higher than that of albumin and globulin in chronic mild?moderate?severe hepatitis.Conclusion:ALB?GLB and A/G is a reference to diagnose patient's liver histopathological level,the level of A/G could better and more sensitively reflect liver histopathoˉlogical level.
