摘 要:目的比较和评价孔径不同的骨钻孔术对关节软骨缺损的远期修复效果。方法用中国白兔20只,在股骨髁关节面制造6mm×8mm全层软骨缺损,分别施行孔直径为1mm和1.5mm的钻孔术,于术后8个月取材做组织学观察,并进行评估。结果(1)1mm孔、1.5mm孔和对照组中,优势修复组织分别为透明软骨、纤维软骨、纤维组织。(2)修复组织厚度:1mm孔、1.5mm孔无显著性差异,接近毗邻软骨厚度。(3)修复组织覆盖缺损的面积:1mm孔>1.5mm孔>对照组。结论软骨下骨钻孔对关节软骨缺损的远期修复效果,1mm孔优于1.5mm孔。Objective To evaluate the long-term effects after subchondral drilling with different diameter of holes.Methods Full-thickness defects (6mm×8mm) were created in the articular surface of both the femoral condyles in 20 Chinese white rabbits. The holes of 1mm and 1.5mm were drilled respectively over the defects with the exception of control groups. They were sampled ancl postoperatively at 8 months. The newly formed tissues in the defects were examined under light and electron microscopes. The long-term effects of repair defect in different groups were compared and evaluated. Results (1) Hyaline cartilage was formed in the group whose diameter of holes is 1mm, fibrous cartilage was formed in the 1.5mm holes one, and the fibrous tissues was found in the control groups.(2) The thickness of repair tissues had no significant difference between the 1mm holes and 1.5mm holes group and were near to the thickness of adjacent articular cartilage.(3)The covered areas of repair tissues of the 1mm holes group exceeded those of the 1.5mm holes and control ones.Conclusion The long-term effects of subchondral bone drilling for repairing full-thickness articular cartilage defects were satisfactory. Long-term effects of 1mm holes is superior to that of 1.5mm holes .
