摘　　要：目的探讨老年人食管癌介入治疗的疗效、临床应用价值和提高疗效的措施。方法本组74例,男48例,女26例,年龄70～86岁,平均76岁。均经X线食管钡餐造影及组织病理学检查证实为鳞癌患者。全部病例均采用Seldinger技术行相应食管节段的供血靶动脉选择性插管和造影,并经导管灌注化疗药物。结果74例患者经介入治疗后肿瘤完全缓解26例,部分缓解39例,无变化8例,进展1例,总有效率87.8%(65/74)。1、2、3年总生存率分别为86.5%(64/74)、39.2%(29/74)和31.1%(23/74)。本组有DSA复查资料的50例中,经治疗后26例(52%)显示肿瘤血管和肿瘤染色完全消失,24例(48%)见肿瘤血管及染色明显减少。本组未见与造影和治疗相关的严重并发症。结论介入疗法治疗老年人食管癌效果显著且技术操作可行,可作为高龄患者的一种重要的甚至首选的治疗方法。Objective To study the curative effect, clinical value and method to improve effect in the inventional therapy of esophageal cancer in old patients.Methods 74 cases(48 males and 26 females ranged in age from 70 to 86 years) with squamous carcinoma diagnosed by histology were treated with selective transcatheter arterial chemotherapy . Results Among 74 cases , 26 cases were achieved complete response ( CR ) , 39 cases were achieved partial response ( PR ) ,8 cases were achieved stable disease and one cases was progressive . The total efective rate was 87.8 % ( 65/74 ). The survival rate of one year, two years and three years reached 86.7%(64/74),39.2%(29/74),31.1%(23/74),respectively. Among them, 50 cases were reexamined with DSA.The vessels and stain of tumour were vanished in 26 cases and decreased in 24 cases. No severe complications were occurred.Conclusion Interventional therapy for old patients with esophageal cancer can be one of the most important methods because its effect is excellent and reliable with little harm.
