国产奥沙利铂联合醛氢叶酸及氟尿嘧啶治疗晚期胃癌临床研究

摘　　要：目的观察国产奥沙利铂(Oxaliplatin,L-HOP)联合大剂量醛氢叶酸(CF)加氟尿嘧啶(5-FU)方案治疗晚期胃癌临床疗效及不良反应,探索晚期胃癌较合理治疗方案。方法本研究采用CF200mg/㎡+5-FU3g/㎡持续滴注48h联合L-HOP130mg/㎡方案治疗65例晚期胃癌,对疗效及不良反应进行评价。结果4例达完全缓解(CR),26例达部分缓解(PR),30例为稳定(SD),5例为进展(PD),有效率(CR+PR)为46.2%(30/65);中位缓解期为7个月。不良反应主要为恶心呕吐、外周静脉炎、脱发、白细胞下降、口腔溃疡、外周神经病变、腹泻等。上述不良反应绝大多数为Ⅰ-Ⅱ度反应,病人可耐受。结论大剂量醛氢叶酸(CF)加氟尿嘧啶(5-FU)静脉持续滴注联合国产奥沙利铂(Oxaliplatin,L-HOP)方案治疗晚期胃癌疗效较好,不良反应较轻,值得临床使用。objective to observe the therapeutic effect and toxicity of high-dose-folinic acid plus 5-fluorouracil bolus and continuous infusion combined with homemade Oxaliplatin(L-HOP) in advanced gastric carcinoma. Methods 65 patients with advanced gastric carcinoma were treated with high-dose-folinic acid plus 5-fluorouracil bolus and continuous infusion 48 hours combined with Oxaliplatin. The effect of treatment, toxicity were observe. Results There were 4 complete responses, 26 partial 30 no changes and 5 progressive disease, in this objective response rate was 46.2%(30/65). The median duration was 7 months, The main side effects include?nausen and vomiting?peripheral phlebitis?alopecia?leukopenia?oral ulcer?peripheral neuritis?diarrhea. All the side effects were tolerable and mild. Conclusions The results showed that high-dose-folinic acid plus 5-fluorouracil bolus and continuous infusion combined with Oxaliplatin may be a safe and effective therapy for the patients with advanced gastric carcinoma, It was worthy of clinical administration.
