摘　　要：目的探讨多发性骨髓瘤患者血浆血小板生成素(Thrombopoietin)及受体基因(C-mpl-mRNA)的变化及其意义。方法选择34例多发性骨髓瘤患者按Durie-Salmon分期分成第Ⅰ期(A组),第Ⅱ期(B组),第Ⅲ期(C组),正常对照组健康成人30例。用夹心酶联免疫吸附法(ELISA)和半定量逆转录-聚合酶链反应(RT-PCR)方法检测对照组及各实验组化疗前后血浆TPO及C-mpl基因表达水平。结果各实验组TPO及C-mpl-mRNA表达均明显高于正常对照组(P<0.05);C组表达明显高于A组与B组(P<0.05);A组与B组间无显著差异。实验组TPO及C-mpl-mRNA表达水平与骨髓瘤分期成正相关(r=0.423,0.436,P<0.05);化疗后TPO及C-mpl-mRNA表达水平显著下降(P<0.05)。结论血浆TPO及C-mpl基因表达水平可在一定程度上反映多发性骨髓瘤病变程度,化疗能降低TPO及C-mpl-mRNA基因表达水平。Objective To study the expression of thrombopoietin (TPO) and its receptor mRNA (C-mpl-mRNA) in multiple myeloma (MM), and to study the correlation between them and the clinical stages. Methods Thirty-four multiple myeloma patients were divided into 3 groups by Durie-Solmon, the stage Ⅰ(A), the stage Ⅱ(B) and the stage Ⅲ(C). The contents of TPO and the mRNA levels of C-mpl in platelets were measured by ELISA and reverse transcriptase polymerase chain reation before and after chemotherapy. Results The TPO and c-mpl mRNA expression levels in patients were significantly higher than those in control group (P< 0.05); those in stage Ⅲ was significantly higher than those in stage Ⅰ and stage Ⅱ (P< 0.05); those in stage Ⅰ was not significantly different from those in stage Ⅱ, those in patients were correlated positively with the clinic stage (r= 0.423, 0.436,P< 0.05), and decrease significantly after chemotherapy (P< 0.05). Conclusion The levels of TPO and c-mpl mRNA expression in patients can be used to reflect the pathological degree of multiple myeloma. and decreased after chemotherapy.
