摘　　要：目的探讨Lotmar干涉视力仪在预测各种类型和不同程度的白内障超声乳化术后视力中的应用价值,为临床决策白内障的手术与否提供依据。方法应用Lotmar干涉视力仪测定患者的干涉条纹视力(IVA),与术后1个月最佳矫正视力(BCVA)进行比较。白内障按类别分为年龄相关性、高度近视性、糖尿病性和其他类;晶状体混浊度按LOCSII分为三级,IVA与BCVA一致为完全符合,相差2行以内为准确,好于2行为假阳性,差于2行为假阴性。结果术后BCVA影响因素的多元回归分析得出术前IVA与之呈显著正线性相关(P<0.01);总准确率为69.51%,年龄相关性组最高(为76%),高度近视性组最低(为60%),假阳性率明显低于假阴性率。结论在能测出IVA的病例中,不论白内障属何种类型,亦不论晶状体混浊度如何,Lotmar干涉视力仪均可较准确预测其术后视力,适宜作为白内障手术前的常规检查方法。Objective To prospectively study and evaluate the application value of the Lotmar visometer in predicting postoperative best corrected visual acuity (BCVA) in cataract patients.Methods Cataract patients who were undergoing phacoemulsification during May～July 2004 were selected for the study. The patient base included 100 eyes with age-related cataract, 40 eyes with high myopia cataract, 26 eyes with diabetic cataract, and 21 eyes with other cataract-related problems. Preoperative visual acuity (VA), IVA, BCVA, lens opacity and the type of cataract were taken as the analysis parameters. Lens opacity was classified according to LOCS Ⅱ and quantified. Four grades were used to classify the cataracts: age-related cataract was graded 0, high myopia cataract was graded 1, diabetic cataract was graded 2, and others was graded 3.Results Multiple regression analysis on postoperative BCVA showed that preoperative IVA was a significant indicator of success in treating cataracts in general (P<0.01); but the type of cataract, lens opacity and VA were not significant indicators. The accuracy of predicting postoperative BCVA by using preoperative IVA was the highest in age-related cataract (76%) and the lowest in high myopia cataract (60%). The false-positive rate (8.56%) was lower than the false-negative rate (21.93%).Conclusion IVA can predict BCVA for various levels of severity and types of cataract, but the factors producing false-positive or false-negative outcome should be avoided. The Lotmar visometer can be used in a routine examination of preoperative cataract.
