摘  要:目的 探讨食管癌浸润转移的SCT征象与MMP 7表达间关系。方法 46例食管癌术前行SCT增强扫描,术后标本采用免疫组织化学法检测肿瘤组织中MMP 7表达。结果 SCT对食管癌邻近结构有无受侵和有无淋巴结转移判断准确率为89.13%(41/46)和86.96%(40/46), 与手术病理结果一致性好(P<0.001)。MMP 7 阳性表达率为60.87%;邻近结构有、无侵犯组及淋巴结有、无转移组MMP 7 的阳性表达率分别为91. 67%, 50. 00%和87. 50%, 46. 67% (P<0.05)。结论 SCT征象可以反映食管癌MMP 7的表达情况及其生物学特性。Objective To investigate the correlation between spiral CT features and expression of MMP-7 in esophageal carcinoma. Methods Spiral CT contrast-enhanced scans were performed in 46 patients with esophageal carcinoma before operation. The specimens were stained with immunohistochemistry. Statistical analysis was performed with SPSS 10.0 software. Results The accuracy of spiral CT contrast-enhanced scan in judging adjacent-infiltration and lymph node metastasis was 89.13% (41 of 46) and 86.96% (40 of 46), respectively (P<0.001). The MMP-7 positive rate was 60.87%. The positive rates of MMP-7 protein were 91.67% and 50.00% respectively in the adjacent-infiltration group and the non-adjacent-infiltration group (P<0.05), while being 87.50% and 46.67% respectively in the lymph node metastasis group and the non-lymph node metastasis group (P<0.05). Conclusion SCT contrast-enhanced scan is a valuable diagnostic technique for the patients of esophageal carcinoma before therapy. The adjacent-infiltration and the lymph node metastasis in SCT features are closely associated with the high expression of MMP-7, indicating that SCT features of esophageal carcinoma can reflect the expression of MMP-7 and the biological characteristic of tumor.
