摘 要:目的探讨偏头痛病人血浆内皮素1(ET1)变化与脑血流动力学变化之间的关系。方法选择偏头痛病例95例,对照组50例,病人中82例行血浆ET1检查,66例行TCD检查,记录血流速度和脉动参数;对同时行TCD检查及血浆ET1检查者的53例偏头痛病人的血浆内皮素和脑血流动力学参数进行了相关研究。结果偏头痛病人血浆ET1水平(80.89±28.70pg/ml)显著高于正常对照(66.55±14.97pg/ml)(P<0.01);偏头痛病人各动脉的血流速度普遍增高(包括Vm、Vs、Vd)(P<0.05);ET1与收缩期血流速度呈正的中度相关,相关系数r=0.4282,P=0.0037。结论偏头痛病人增高的ET1可能是导致脑血流动力学发生变化的原因之一。Objective: To investigate the relation between the changes of cerebral hemodynamics using Transcranial Doppler Sonography (TCD) and serum endothelin-1(ET-1). Methods: 95 migraineurs and 50 controls were chosen and TCD were performed in 66 of total 95 patients to record the blood velocity and pulsating parameter; the level of serum ET-1 was measured in 82 of 95 patients. For 53 patients with both TCD and serum ET-1 examination, a correlation analysis between the serum ET-1 and various TCD parameters was made. Results: The level of serum ET-1 in migraineurs(80.89±28.70pg/ml) is significantly higher than that of controls(66.55±14.97 pg/ml) (P<0.01); the blood flow velocity including Vm, Vs, Vd in every arteries in migraineurs is also higher than that of controls. There is linear correlation between serum ET-1 level and Vs (velocity in systole), and the correlation coefficient is 0.4282 (P=0.0037). Conclusions: The results suggested that the increase in the level of serum ET-1 is one of the reasons that resulting in the changes of cerebral hemodynamics simultaneously in migraineurs.
