舒芬太尼、芬太尼用于颌面外科术后镇痛的对比观察

摘　　要：目的研究舒芬太尼用于病人自控静脉镇痛(PCA)的效果和安全性。方法70例颌面外科全麻术后病人随机分为舒芬太尼(Suf)组和芬太尼(Fen)组,各35例。分别使用舒芬太尼或芬太尼进行PCA,不给负荷量。记录术后0h、4h、8h、12h、24h、48h各时间点的疼痛评分、镇静评分,PCA次数、并发症和病人的满意度等。结果Suf组8h和24hVAS评分明显低于Fen组(P<0.05),其它点无显著性差异(P>0.05);Suf组各时间点镇静评分均明显大于Fen组(P<0.05);Suf组PCA按压次数显著低于Fen组(P<0.05);两组并发症发生率低,无统计学差异(P>0.05);Suf组病人对PCA满意率显著高于Fen组(P<0.05)。结论舒芬太尼用于口腔颌面外科术后PCA,镇痛安全有效,并且镇静效果优于芬太尼。Objective:To investigate the clinical efficacy and safety of sufentanil for patients with controlled intravenous analgesia ( PCA) after maxillofacial surgery. Methods: 70 patients were randomly divided into sufentanil (Suf) group (n=35) and fentanyl (Fen) group (n=35). Sufentanil 50 μg or fentanyl 0.5 mg was used respectively via PCIA and no loading dose was administered. Visual analogue scale (VAS) and sedation score were recorded or 4h,8h,12h,24h,48h after PCA. The change of circulation and respiration, the push times of PCA, the complication and the patients' satisfactory degree were also observed. Results: In the Suf group, VAS scores in 8 and 24h were significantly lower than that in Fen group(P<0.05).The sedation scores in the Suf group were significantly higher than that in Fen group(P<0.05). Compared with Fen group, the number of PCA was lower in Suf group(P<0.05). The incidence of nausea, vomiting in two groups was low and had no significant difference(P>0.05).The patients' satisfactory rate with PCA was higher in Suf group than that in Fen group (P<0.05). Conclusion: Sufentanil is an effective and safe analgesic method after maxillofacial surgery and its sedate effect is superior to that of fentanyl.
