摘 要:目的:观察24小时脑电动态监测与常规脑电图对癫痫诊断和鉴别诊断的价值。方法:195例患者进行常规脑电图1~2次,然后行24小时动态脑电监测,监测同时做视反应、过度换气等诱发实验。结果:有二人判定,分为正常、界限、轻度异常、中度异常、重度异常和局限性异常,同时判断有无癫痫样放电。结果:癫痫患者105例常规脑电异常37例(35.2%),而24小时动态监测异常为56例(53.33%);可疑癫痫50例中常规脑电异常20例(40%),监测异常33例(66%);晕厥患者40例中常规脑电异常10例(25%),而监测异常为15例(37.5%)。结论:24小时动态脑电监测比常规脑电图更易发现异常,对癫痫的诊断和鉴别诊断具有重要的价值。Objective: To observe the value of 24h ambulatory electroencephalography (EEG) and routine EEG in diagnosis and differential diagnosis of epilepsy. Methods: Routine EEG were performed 1~2 times in 195 patients, then followed by 24h AEEG. The provocative test such as visual response and hyperventilating were also performed at the same time. The results were classified as normal, boundary, mild abnormal, moderate abnormal and marked abnormal EEG. Two doctors reported all results respectively; the epileptiform discharge was also noted. At last, the reports were analyzed. Results: Abnormal discharge was found in 37 (35.2%) of 105 epileptic patients' routine EEG, but 56(53.33%) patients were abmormal in 24h aEEG. In the 50 dubious epileptic patients, 20 (40%) patients' routine EEG and 33 (66%) patients' 24h AEEG showed abnormal discharge. In the other 40 syncopic patients, abnormal discharge was 10(25%) case in routine EEG and 15(37.5%) case in 24h AEEG .Conclusions: In founding the abnormal discharge, 24h AEEG was easier than routine EEG. The 24h AEEG was important valuable in diagnosis and differentiation diagnosis of epilepsy.
