血清GCA、CA19-9和IAP联合检测对胃癌手术前后动态观察的价值

摘　　要：目的　探讨血清中GCA、CA19- 9、IAP对胃癌诊断和动态观察中的价值。方法　检测经病理确诊的 34例胃癌病人术前术后血清中的GCA、CA19- 9、IAP含量及其动态变化。同时检测了 15 6例正常人进行对照分析。结果　此三项指标对胃癌诊断的敏感性和特异性分别为 76 .5 %、82 .4%、5 3 .1%和 91.7%、78%、5 4.6 %。三项联合检测敏感性和特异性分别为 91%和 10 0 %。手术前后动态检测显示 ,满意的手术患者术后三项指标正常。对术中有较大癌残留者手术后三项指标测定值下降未能达正常值 ,复发者其值亦升高。结论　GCA、CA19- 9、IAP是对胃癌的诊断和术前、术后监测较好的肿瘤标志物 ,联合检测能大大提高敏感性及特异性 。Objective To investigate the diagnostic and prognostic value of combined measurement of serum GCA,CA19-9 and IAP before and after operation for patients with gastric cancer.Methods Serum GCA,CA19-9 and IAP in 34 cases of patients with gastric cancer before and after operation were measured.The three markers were tested in 156 cases of normal control simultaneously.Results The sensitivity and specificity of serum GCA,CA19-9 and IAP for the diagnosis of gastric cancer were 76.5%,82.4%,53.1% and 91.7%,78.0%,54.6%.The sensitivity and specificity of combined measurement of the three markers were 91% and 100%.The level of the three markers were reduced to normal if tumor was removed completely,and the level were not able to reduce to normal if removed incompletely.At the same time,the level of the three markers remained high in patients with relapse.Conclusion GCA,CA19-9 and IAP were good markers for the diagnosis and prognosis at gastric cancer and combined measurement can increase the sensitivity and specificity significantly,its value to apply for the cliuic.
