乾坤胶囊结合高能聚焦超声对原发性肝癌患者免疫功能的影响

摘　　要：目的研究乾坤胶囊结合高能聚焦超声对原发性肝癌患者的免疫功能影响,并探讨该复方中药制剂抗肝癌的作用机理。方法对90例经细胞病理学证实的原发性肝癌患者,随机分为治疗组30例,对照组30例,空白组30例。从T细胞亚群变化(CD+3、CD+4、CD+8、CD+4CD+8)、NK(自然杀伤细胞)、LAK(淋巴因子激活的杀伤细胞)活性、IL2(白细胞介素2)水平、TGFα(转化生长因子α)的含量变化等方面进行了前瞻性对比治疗研究,并与15例健康人群作比较。结果原发性肝癌患者T细胞亚群紊乱,NK细胞活性、LAK细胞活性、IL2水平低下,TGFα的含量明显升高。与健康人群比较差异有显著性(P<0.01及P<0.05),治疗后各组观察指标有不同程度的回升或降落,与治疗前比较差异有显著性(P<0.01及P<0.05)。治疗后治疗组、空白组的NK细胞活性、LAK细胞活性、IL2水平明显升高,TGFα的含量则显著降低,与对照组比较均有显著性差异(P<0.01及P<0.05);CD+3、CD+4、CD+4CD+8则治疗组优于对照组及空白组(P<0.01及P<0.05)。空白组优于对照组(P<0.05)。结论原发性肝癌患者免疫功能明显低下,乾坤胶囊结合高能聚焦超声可逆转原发性肝癌患者的免疫功能受抑制状态,增强NK细胞、LAK细胞活性,升高IL2水平,降低TGFα的含量。Objective:To investigate the effect of Qiankun capsule combined with high-intensity focused ultrasound on the immunological function of patients with primary liver cancer,and to study its mechanism of anti-tumor action. Methods: 90 patients with primary liver cancer identified by cytopathology were randomly divided into three groups,30 cases in treatment group, 30 cases in control group and 30 cases in blank group. The patient's T lymphocyte subpopulation (CD+_3, CD+_4, CD+_8, CD+_4/CD+_8), NK activity,LAK activity, levels of IL-12,TGF-αwere retrospectively analyzed and compared with those of 15 healthy subjects. Results: The T lymphocyte subpopulation in patients with primary liver cancer was disorder; NK activity,LAK activity and levels of IL-12 were decreased; the content of TGF-αwas obviously increased,as compared with those of healthy subjects (P<0.01 or P<0.05). After treatment these parameters were significantly increased or decreased at different degrees, as compared with those before treatment (P<0.01 or P<0.05). After treatment NK activity,LAK activity and levels of IL-12 in treatment group and blank group were obviously increased,however, TGF-α was significant decreased,as compared with those of control group (P<0.01 or P<0.05). The CD+_3, CD+_4, CD+_8, CD+_4/CD+_8 in treatment group were superior to those in control group and blank group (P<0.01 or P<0.05),and which in blank group were superior to those in control group (P<0.05). Conclusion: Qiankun capsule combined with high-intensity focused ultrasound can enhance the immunological function of patients with primary liver cancer,which can increase NK activity,LAK activity and levels of IL-12 and decrease content of TGF-α.The effect of Qiankun capsule on prognosis of patients with primary liver cancer is related to the increase of immunological function.
