摘　　要：目的 研究不同PEEP的机械通气对肺部炎症介质反应的影响。方法 15只普通健康家猪随机分为3组:A组(V_T=10ml/kg,PEEP=0cmH_2O),B组(V_T=10ml/kg,PEEP=8cmH_2O),C组(V_T=10ml/kg,PEEP=16cmH_2O)。机械通气3h后放血处死,分别取肺叶组织做病理学检查及测定ICAM—1表达水平。结果 光镜下A、C两组有肺损伤表现,C组最明显,B组则无肺损伤表现。A组ICAM—1的表达为弱阳性,B组ICAM—1的表达为阴性,C组ICAM—1的表达为强阳性。结论 适当应用PEEP可预防机械通气引起的肺炎症反应,而过高的PEEP则加重肺部炎症反应。Objective To inuestigate the influence of PEEP on pulmonary inflammatory mediator response. Methods Fifteen healthy pigs with continuous mechanical ventilation were randomly divided into group A(Vr = 10ml/ kg, PEEP = 0cmH2O) ,), group B(VT = 10ml/ kg, PEEP= 8cmH2O) and group C(Vr = 10ml/ kg, PEEP= 16cmH2O), The pulmonary histopathological alteration was observde after 3 hours' . Immunohistochemistry was used to evaluate the expression of ICAM - 1 protein in lung tissues as well. Results There in an injurious his-tological change in group A and C, whereas no significant alteration in group B. The expression of ICAM - 1 protein in lungs was positive in group A and C, while negative in group B. Conclusions Suitable PEEP could mediator protect pulmonary inflammatory responses.
