产妇的分娩满足感与产后抑郁症

摘　　要：目的:了解产妇的分娩满足感及影响因素,探讨其与产后抑郁症的相关性。方法:采用单纯随机抽样方法抽取2004-07-03/2004-12-31在中国医科大学附属第二临床学院妇产科自然分娩的产妇103人(初产妇95人,经产妇8人)。均为无分娩期并发症、产科合并症和并发症的健康产妇,且自愿参加。产后第2～5天通过访问的方式,将产妇对分娩过程的回顾进行笔录,对产妇分娩的满足感进行评分。产后30d电话访问产妇,填写爱丁堡产后抑郁量表。对笔录内容由临床专家组采用双盲法进行质的分析,多次阅读访问记录并从中抽出精句和意见,最后取得被访者确切的见解及对症得出结论。研究方法本身不是找出大量数据,而是由这些典型的例子来代表大多数人的经历。结果:按意向处理分析,进入结果分析产妇103人。①影响分娩体验中满足感的相关因素:医务人员提供的信息和技术水平,产妇的自我评价,陪伴者的作用,是否有分娩的真实感,环境的影响。②实际分娩体验:比预想的好产妇78人中,分娩有满足感78人(100%);比预想的差产妇25人中,分娩有满足感10人(40%),无满足感15人(60%)。产妇分娩满足感自评表总分与产后抑郁量表总分的关系:两者呈显著负相关(80.48±18.43,8.90±5.66,r=-8.68,P<0.01)。结论:分娩过程各环节对产妇的满足感均有影响;AIM:To know the delivering satisfactory feeling and the influencing factors in parturiens,and investigate its correlation with postpartum depression. METHODS:A total of 103 parturiens (95 primiparas and 8 pultiparas),who deliver ed naturally in the Department of Obstetrics and Gynecology,Second Clinical Coll ege,China Medical University between July 3rd and December 31st 2004,were select ed with the method of simple random sampling.All the subjects were healthy partu riens without no complication in delivery,obstetric complications, and they all participated in the study voluntarily.By means of visiting, the review of the de livery process from the parturiens were recorded in written form on the 2nd to 5 th days postpartum, and their delivering satisfactory feeling was scored.The par turiens were visited on telephone to fill the Edinburgh postnatal depression sca le 30 days postpartum.The quality of the written record was analyzed by the clin ical specialists with the double blinded method,the records were read for many t imes,and classic sentences and suggestions were extracted, finally the definite opinions from the subjects were obtained,and drew a conclusion according to the symptoms.The investigative method itself was not to find out plenty of data, but aimed to represent the experience of most people by these classic examples. RESULTS:According to intention to treat anlaysis, all the 103 parturiens wer e involved in the analysis of results.①The related factors that affected the sa tisfactory feeling in delivery were the levels of the information and technique provided by the medical staff,self assessment of the parturiens,role of the com panion,having the sense of reality for delivery or not,and the environmental inf luence;②Actual delivering experience: It was better than expectation in 78 case s,and 78 cases(100%) had satisfactory feeling in delivery;It was worse than exp ectation in 25 cases,10 cases(40%) had satisfactory feeling in delivery,and 15 cases(60%) had no satisfactory feeling in delivery;�
