摘　　要：目的探讨尿中细胞角蛋白19片段(CYFRA211)和脱落细胞学联合检测对尿路上皮肿瘤的诊断价值和术后监测的意义。方法应用酶联免疫方法(ELISA)检测30例尿路上皮肿瘤患者和20例非尿路上皮肿瘤患者尿液CYFRA211的表达并同时进行尿液脱落细胞学检测。结果尿路上皮肿瘤组CYFRA211阳性表达率明显高于对照组(P<0.01),G1CYFRA211阳性表达率明显低于G2、G3(P<0.05),Ta～T1CYFRA211明显低于T2～T4(P<0.05),G1尿液脱落细胞阳性率明显低于G2、G3(P<0.05),Ta～T1尿液脱落细胞阳性率明显低于T2～T4(P<0.05)。结论CYFRA211和脱落细胞学联合检测可用于尿路上皮肿瘤的早期诊断和术后监测。Objective To study the individual and combined urinary CYFRA21-1,exfoliated cells in urine from patient to evaluate their clinical significance for early diagnosis and postoperative follow-up of urothelial tumor.Methods CYFRA21-1 in the urine of 50 patients including 30 with urothelial tumor.20 with another urological pathology or free of urothelial disease were detected with enzyme linked immunosorbent assay(ELISA). At the same time,urinary cytology was examined.Results The positive rate of CYFRA21-1 in urothelial tumor was significantly higher than that in no urothelial tumor(P<0.01).The positive rate of CYFRA21-1 for low grade(G_1 )low stage(T_a ~T_1)was significantly lower than that for high grade(G_2~G_3) high stage (T_2~T_4) (P<0.05);The positive rate of the exfoliative cytology for low grade (G_1 ) low stage (T_a ~T_1)was significantly lower than that for high grade(G_2~G_3) high stage(T_2~T_4) (P<0.05).Conclusion The combinative examination of urinary CYFRA 21-1 and urinary cytology could be used for the early noninvasive methods in diagnosing and monitoring urothelial tumor.
