消化系恶性肿瘤晚期患者胰岛素抵抗和胰岛素分泌指数变化与糖代谢障碍的关系

摘　　要：目的探讨恶性肿瘤患者胰岛素抵抗和胰岛素分泌指数变化在糖有效利用障碍中的作用机制,为临床营养治疗提供实验依据。方法测定45例健康体检正常者和45例消化系恶性肿瘤晚期患者空腹血糖、空腹胰岛素、空腹乳酸和服糖后血糖含量,并采用稳态模式评估法计算胰岛素抵抗指数和胰岛素分泌指数。结果恶性肿瘤晚期组空腹乳酸、空腹胰岛素、胰岛素抵抗指数及服糖后血糖含量均明显高于正常对照组(P均<0.01),恶性肿瘤组胰岛素分泌指数明显低于正常对照组(P<0.01)。恶性肿瘤组空腹血糖与正常对照组比较无显著性差异(P>0.05)。结论恶性消化系肿瘤患者存在糖代谢紊乱并与胰岛素抵抗和胰岛素相对血糖分泌功能有关。Objective To explore the effects of insulin resistance and insulin excretion on the disorder of glucose-metabolism in the patients with advanced enteron malignant tumors.Methods The levels of fasting glucose,insulin,lactic acid,postprandial glucose,lactic acid of both the control group (n=45)and the patients with malignant tumor(n=45)were measured,and the indexes of HOMA-IR and HOMA-β of the the patients with malignant tumors and the controls were calculated.Results The levels of the fasting plasma lactic acid and the postprandial plasma glucose and lactic acid in terminal patients with malignant tumors were higher significantly than that in the controls(P<0.01);There was no significant difference of the fasting glucose between the two groups(P>0.05).Conclusion The insulin resistance and the hyposeretion of insulin compared with the glucose play an important role in the disorder of glucose-metabolism of the terminal patients with advanced enteron malignant tumor.
