摘  要:目的:探讨马来酸桂哌齐特对豚鼠噪声性听力损失的保护作用。方法:实验于2003-10/11在河北医科大学第三医院耳鼻喉科实验室进行。取22只豚鼠随机分成3组:马来酸桂哌齐特组、生理盐水组各10只,正常组2只。马来酸桂哌齐特组及生理盐水组豚鼠分别腹腔注射马来酸桂哌齐特20mg/(kg·d),生理盐水4mL/(kg·d),注射后15min行白噪声(110dB)暴露,60min/d,连续9d,正常组豚鼠用以制备观察耳蜗螺旋器毛细胞正常形态的电镜标本,无需任何处理。前两组在暴露后不同时间(1,3,5,7,9d)分别测试听脑干反应,并用扫描电镜观察耳蜗毛细胞的结构变化。结果:22只动物全部进入结果分析。①扫描电镜观察显示,正常组豚鼠耳蜗内、外毛细胞形态正常;生理盐水组耳蜗内、外毛细胞均有听毛紊乱、融合及缺失;马来酸桂哌齐特组耳蜗病变比生理盐水组轻,听毛仅有轻微倾倒、融合现象。②无论任何时段马来酸桂哌齐特组豚鼠听脑干反应阈移小于生理盐水组(P<0.01);马来酸桂哌齐特组豚鼠在噪声暴露后第7天听脑干反应阈值已接近正常[(9.18±3.47),(6.93±3.24)dB,P>0.05],而生理盐水组豚鼠在噪声暴露后第9天听脑干反应阈值与正常组相比仍有显著差异[(16.84±3.78),(7.82±3.01)dB,P<0.05]。结论:马来酸桂哌齐特能够减轻噪声对耳蜗毛细胞的损害,对噪声性听力损伤具有较明显的保护作用。AIM:To study the protective effect of cinepazide maleate on noiseinduced hearing loss in guinea pigs.METHODS:The experiment was performed in the Department of Otorhinolaryngology, Third Hospital of Hebei Medical University from October to November 2003.A total of 22 guinea pigs were divided randomly into cinepazide maleate group(received 20 mg/kg cinepazide maleate per day, n=10),saline group (received 4 mL/kg saline per day,n=10) and control group(n=2).Fifteen minutes after injection,guinea pigs were exposed to white noise at 110 dB for consecutively 9 days(60 minutes per day). Auditory brainstem response thresholds were measured in different time after exposure(1,3,5, 7 and 9 days),The structural changes of cochlea hair cells were also observed by using a scanning electron microscope.RESULTS:All 22 guinea pigs were involved in the result analysis.①Scanning electron microscope showed that hear hair of the inner and outer hair cells displayed normal in control group,mess,fusion and imperfections in saline group,and slight mess and fusion in cinepazide maleate group.②The threshold values of auditory brainstem response shifts in cinepazide maleate group were significantly fewer than those in the control group(P< 0.01). Auditory brainstem response threshold almost recovered in cinepazide maleate group 7 days after the exposure[(9.18±3.47), (6.93±3.24) dB,P >0.05];however, there was significant difference between saline control group and normal control group at 9 days after the exposure[(16.84±3.78),(7.82±3.01) dB,P< 0.05]. CONCLUSION:Cinepazide maleate is able to reduce damage of hair cells, and it can significantly protect against noiseinduced hearing loss.
