摘  要:目的探讨建立标准化猪急性心肌梗死模型方法。方法将27只中国实验用小型猪随机分成实验组及对照组,分别结扎左冠状动脉前降支上等流量点及中下1/3处。术中监测血流动力学指标;术前、术后1周、5周磁共振检查心脏结构及射血分数,5周后计算全心梗死体积。结果结扎点血流量与梗死体积及左心室EF值变异呈正相关。5周后dp/dt、左心室收缩末期及舒张末期内压(LVSBP及LVDBP)均显著降低(P<0.01)。两组心肌梗死体积(484.22±225.88)mm3比(2986.34±1937.13)mm3,P<0.01)、EF及dp/dtmax的变异差异均有统计学意义(P<0.05)。结论结扎LAD上等流量点可建立心肌梗死体积变异和心功能变异较小的急性心肌梗死动物模型。Objective To establish a standard swine model with equal infarct size and heart function decrease theoretically by ligating LAD at the equal coronary blood flow rate point.Methods Twenty-seven Chinese experimental mini-pigs were divided into experimental group (n=12) and control group (n=15).The whole length of left anterior descending coronary artery (LAD) was dissected and the blood flow rate recorded.LAD was ligated at the point of (25.06± 2.50) ml/min and the mid-inferior 1/3 point respectively.All animals underwent serial hemodynamic study immediately followed by magnetic resonance image (MRI).The infarction size was determined by tetrazolium staining at the 5th week.Results The mortality was 16.7% (2/12) and 33.3% (5/15) in the experimental and control groups respectively.There were significant differences in the variations of the infarction size [(484.22± 225.88) mm3 vs (2 986.34± 1 937.13) mm3, P< 0.01],dp/dt max (P< 0.01),and ejection fraction (EF, P< 0.05) of left ventricle between two groups.There were significant differences in dp/dt [Max: (1 546.67± 477.89) mm Hg/s vs (973.77± 208.12) mm Hg/s, P< 0.01],and left ventricle pressure [SBP (75.56± 12.59) mm Hg vs (59.03± 6.93) mm Hg, P< 0.01;DBP: (6.56± 3.14) mm Hg vs (4.22± 1.41) mm Hg, P< 0.05] before and after LAD ligation in the experimental group 5 weeks later.There was high relativity between the blood flow rate and the infarct size (r= 0.752; P< 0.01),and EF variation (r= 0.641 90; P< 0.05).Conclusion Blood flow rate based LAD ligation may establish a reliable,stable acute myocardial infarction model with equal infarct size and heart function decrease for the pre-clinical study of cardiac regeneration.
