高龄高度梗阻食管癌患者基因Survivin、Fas和FasL表达与预后的关系

摘　　要：目的:探讨高龄高度梗阻食管癌患者基因Survivin、Fas、FasL的表达与预后的关系。方法:对88例75～92岁高梗阻食管癌患者行内镜治疗,其中42例行内镜下单纯支架置入术,46例联合微波组织凝固、局部注药治疗,同时采用免疫组织化学法检测肿瘤组织的基因Survivin、Fas、FasL的表达。结果:88例患者治疗后进食均显著改善。联合治疗组术后生存时间较单纯支架组明显延长,再狭窄率显著降低(P<0.05),肿瘤组织基因Survivin、Fas、FasL的阳性表达率分别为73.86%、46.59%、54.55%。Survivin和FasL的表达与患者术后生存时间呈负相关(P<0.05)。结论:基因Survivin、FasL可作为预测预后的重要参考指标,其高表达提示食管癌患者预后不良。Objective: To explore the prognostic importance of Survivin, Fas and FasL expressions in aged patients missed operation and radiotherapy with severe obstructive esophageal carcinoma. Methods: Eighty-eight aged patients with esophageal carcinoma received the endoscopic therapy,of them, 42 were treated with memory metallic stent placement only and 46 were treated with microwave tissue coagulation (MTC) and local chemotherapy in addition. Specimens from 88 patients with squamous cell carcinoma of the esophagus were taken for immunohistochemical evaluation of Survivin, Fas and FasL. Results: After treatment, all the patients' conditions of ingesting food were improved obviously. The survival time of patients received combining therapy was longer than that of those with stent placement only, and the rate of recurrent obstruction was lower in combining group(P<0.05). The rate of Survivin,Fas and FasL expression in esophageal cancer tissues was 73.86%,46.59% and 54.55%, respectively. The mean survival for patients with high Survivin and FasL expressions was less than that for patients with low expression. Conclusion: High expressions of Survivin and FasL may provide an unfavorable prognosis in patients with esophageal cancer.
