磷脂酶A_2和溶血磷脂酸在卵巢癌诊断中的价值

摘　　要：目的探讨分泌性磷脂酶A2(sPLA2)和溶血磷脂酸(LPA)对卵巢癌诊断、病程监测和预后预测的价值。方法用3H标记大肠杆菌膜为底物的液闪方法和比色法,分别对初发或复发卵巢癌和卵巢良性肿瘤患者各30例进行血液中sPLA2活性和LPA水平检测,并与乳腺癌患者作比较,同时对手术前后卵巢癌患者进行动态检测,以评价两项指标在卵巢癌检测中的敏感性和特异性。结果初发卵巢癌组与卵巢良性肿瘤组相比,sPLA2活性(24.47±5.64U/L与4.94±1.86U/L)和LPA水平(6.35±2.32与1.92±0.75mmol/L)差异均具有统计学意义(均P<0.01)。复发卵巢癌组与卵巢良性肿瘤组相比,sPLA2活性(26.53±6.05U/L与4.94±1.86U/L)和LPA水平(7.03±2.77mmol/L与1.92±0.75mmol/L)差异也均具有统计学意义(均P<0.01)。初发卵巢癌组手术前后sPLA2活性和LPA水平与时间变化相关(分别r=0.88,r=0.81,均P<0.05)。sPLA2活性和LPA水平的敏感性分别为100%和95%,特异性分别为73.3%和83.3%。初发和复发卵巢癌与初发和复发乳腺癌相比,LPA水平差异有统计学意义(P<0.01)。结论sPLA2活性和LPA水平可能对卵巢癌的诊断、治疗监测和预后评价有一定的价值。Objective To explore the diagnostic value of secretory phospholipase A2 (sPLA2) and lysophosphatic acid (LPA) for ovarian cancer and evaluate the correlation of sPLA2 activity and LPA level to the prognosis in the patients with ovarian cancer.Methods ~3H oleic acid labeled E. coli membrane and spectrophotometry were used to detect the original and recurrent ovarian cancer. The motive observation between sPLA2 activity and LPA level was done.At the same time, The sensitivity and specificity for the diagnosis of ovarian cancer were analyzed.Results Comparing the original ovarian cancer group with the ovarian benign tumor group, the difference between sPLA2 activity (24.47 ±5.64 U/L,4.94 ±1.86 U/L) and LPA levelc(6.35 ±2.32 μmol/L,1.92 ±0.75 μmol/L) were significantly (P<0.01). Comparing recurrent ovarian cancer group with the ovarian benign tumor group, the difference between sPLA2 activity (26.53 ±6.05 U/L, 4.94 ±1.86 U/L)and LPA level(7.03 ±2.77, 1.92 ±0.75) was statistically (P<0.01). The correlation between sPLA2 activity and LPA level to the pre and post operation were analyzed, respectively (P<0.01, P<0.05) in the diagnosis of ovarian cancer.Conclusions It suggested that sPLA2 activity and LPA level have important value in diagnosis, post operations monitor and prognosis of the patients with ovarian cancer.
